“Heart Shaped Sign” in Bilateral Medial Medullary Infarction

บทคัดย่อ

ผู้นิพนธ์นำเสนอรายงานผู้ป่วยชายอายุ 70 ปี ซึ่งเกิดภาวะที่มีการเปลี่ยนแปลงของระดับความรู้สึกตัว โดยตรวจร่างกายพบหนังตาตกลงทั้ง 2 ข้าง ตากระตุกแบบ vertical nystagmus การกลอกตาผิดปกติทั้ง horizontal และ vertical direction และอ่อนแรงของแขนและขาทั้ง 2 ข้าง (quadriparesis) เอกซเรย์คลื่นแม่เหล็กไฟฟ้าของสมองพบมี hyposignal intensity area on T1-weighted, hypersignal intensity on T2-weighted, fluid-attenuated inversion recovery (FLAIR) and restrict diffusion-weighted image (DWI), low signal in apparent diffusion coefficient image (ADC) in the bilateral upper part of medulla oblongata extending to the pontomedullary junction. Contrast-enhanced magnetic resonance angiography (MRA) of brain พบมี irregular narrowing and atherosclerotic change of vertebral artery ทั้งสองข้างโดยเป็นมากที่หลอดเลือดข้างซ้ายมากกว่าข้างขวา รวมทั้งพบมี atherosclerotic change of both middle cerebral arteries, both posterior cerebral arteries and basilar artery วินิจฉัยโรคเป็นacute bilateral medial medullary infarction (MMI) โดยที่มีรูปร่างของสมองขาดเลือดคล้าย “รูปหัวใจ” ผู้ป่วยได้รับการรักษาโดยใช้ยา aspirin, ยาลดระดับไขมันในเลือด atorvastatin รวมทั้งได้รับยาปฏิชีวนะเพื่อรักษาภาวะ aspiration pneumonia ฝึกกายภาพบำบัด ให้อาหารทางสายยางผ่านทางการเจาะหน้าท้องและต้องเจาะคอเนื่องจากไม่สามารถหายใจเองได้และต้องใช้เครื่องช่วยหายใจเป็นเวลานาน หลังติดตาม การรักษาผู้ป่วยยังคงมีอาการแขนขาที่ยังอ่อนแรงและช่วยเหลือตัวเองไม่ได้
