ปัจจัยที่มีผลต่อการเสื่อมสภาพของสไลด์แก้วอิมมูโนฮีสโตเคมี HER2 ในผู้ป่วยมะเร็งเต้านม

บทคัดย่อ

บทนำ: การทบทวนสไลด์แก้วอิมมูโนฮีสโตเคมี HER2 เป็นปัจจัยสำคัญในการวางแผนการรักษามะเร็งเต้านม การนำสไลด์เก่ากลับมาทบทวนซ้ำในผู้ป่วยที่ส่งต่อจึงพบได้บ่อย แต่หากสไลด์เสื่อมสภาพอาจทำให้การประเมินคลาดเคลื่อน และกระทบต่อการวางแผนการรักษาแบบใช้ยามุ่งเป้า (Targeted therapy)

วัตถุประสงค์: เพื่อศึกษาปัจจัย และระยะเวลาที่มีผลต่อการเสื่อมสภาพของสไลด์แก้วอิมมูโนฮีสโตเคมี HER2 ในผู้ป่วยมะเร็งเต้านมที่เก็บรักษาไว้นาน 1-5 ปี

ระเบียบวิธีวิจัย: การศึกษาเชิงวิเคราะห์ย้อนหลังในสไลด์แก้วอิมมูโนฮีสโตเคมี HER2 ในผู้ป่วยมะเร็งเต้านมที่มีระดับ 3+ ของโรงพยาบาลนครพิงค์ ระหว่าง มิถุนายน 2563 – ธันวาคม 2568 จำนวน 154 สไลด์ ประเมินความถูกต้องซ้ำการติดสีครบวงและความเข้มของสี โดยพยาธิแพทย์ 3 ท่าน วิเคราะห์ปัจจัยที่เกี่ยวข้องด้วยสถิติ Logistic Regression

ผลการศึกษา: จากสไลด์แก้วทั้งหมด 154 สไลด์ พบว่ายังคงสภาพจำนวน (105 สไลด์ ร้อยละ 68.18) โดยส่วนใหญ่เป็นสไลด์ที่มีอายุ < 1 ปี (37 สไลด์ ร้อยละ 35.24 ของกลุ่มที่ยังคงสภาพ) และส่วนใหญ่ได้จากกระบวนการเก็บชิ้นเนื้อแบบ Core needle biopsy (94 สไลด์ ร้อยละ 89.52) เมื่อประเมินสัดส่วนการติดสีครบวง และความเข้มของสี พบว่าสไลด์แก้วที่มีอายุมากกว่า 1 ปีขึ้นไปมีการคงสภาพลดลงอย่างต่อเนื่อง Crude OR 0.70 (95% CI 0.58 – 0.85, p < 0.001) โดยสไลด์แก้วอายุ > 4-5 ปี มีการคงสภาพลดลงอย่างมีนัยสำคัญ Crude OR 0.09 (95% CI 0.03 - 0.33, p < 0.001) ในขณะที่ขนาดของชิ้นเนื้อไม่มีผลต่อการคงสภาพเมื่อควบคุมปัจจัยด้านระยะเวลา

สรุปผล: สไลด์แก้วที่จัดเก็บนานเกิน 4 ปี มีความน่าเชื่อถือในการประเมินระดับ HER2 ลดลงอย่างมีนัยสำคัญ จึงควรพิจารณาตรวจอิมมูโนฮิสโตเคมี ซ้ำจากบล็อกพาราฟินแทนการทบทวนจากสไลด์เดิม
