ที่มา: ภาวะโรคต้อกระจกเป็นสาเหตุหลักของภาวะตาบอดในประชากรไทย โรคนี้มีผลกระทบต่อคุณภาพชีวิตและเศรษฐกิจของประเทศ โดยส่วนใหญ่มักพบโรคต้อกระจกในกลุ่มผู้สูงอายุ การผ่าตัดต้อกระจกและการใส่เลนส์เทียมมีความสำคัญในการเพิ่มศักยภาพทางการมองเห็นและลดการตาบอดที่มาจากโรคต้อกระจก
วัตถุประสงค์: เพื่อศึกษาประสิทธิภาพของการผ่าตัดต้อกระจกด้วยเทคนิค Extracapsular cataract extraction (ECCE) เปรียบเทียบกับการผ่าตัดต้อกระจกด้วยเทคนิค Phacoemulsification (Phaco) ที่โรงพยาบาลศรีสังวาลย์ในจังหวัดแม่ฮ่องสอน
วิธีการศึกษา: รูปแบบการวิจัยและเก็บข้อมูลแบบ Prospective and non-randomised design ระหว่างกลุ่มที่ได้รับการผ่าตัดต้อกระจกระหว่างเทคนิค ECCE และ เทคนิค Phaco ในโรงพยาบาลศรีสังวาลย์ เก็บข้อมูลระหว่าง 1 มิถุนายน พ.ศ.2566 ถึง 31 ธันวาคม พ.ศ.2566 โดยทำการสุ่มตัวอย่างแบบเฉพาะเจาะจง (purposive sampling) กลุ่มละ 34 คน การวิเคราะห์ข้อมูลใช้ Independent t-test และ Chi-square test โดยใช้โปรแกรม SPSS version 26
ผลการศึกษา: ผู้ป่วยต้อกระจกทั้งหมด 68 ราย (34 ราย ECCE และ 34 ราย Phaco) อายุระหว่าง 71-80 ปี, 61-70 ปีมีสัดส่วน ร้อยละ 33.8 และ 32.3 ตามลำดับ เพศชายร้อยละ 47.1 และเพศหญิงร้อยละ 52.9 ผ่าตัดข้างขวาร้อยละ 50 ระดับการมองเห็นก่อนผ่าตัดมากกว่าหรือเท่ากับ severe visual loss ทุกรายทั้ง 2 กลุ่ม ผลการเปรียบเทียบการมองเห็นหลังผ่าตัดพบว่า กลุ่มที่รับการผ่าตัดด้วยเทคนิค Phaco มีสัดส่วนการมองเห็นดีกว่ากลุ่มที่ใช้เทคนิค ECCE ที่ระยะเวลาหลังผ่าตัด 1 วัน 1 สัปดาห์ 4 สัปดาห์ 8 สัปดาห์ อย่างมีนัยสำคัญทางสถิติที่ p= 0.004 p<0.001 p<0.001 และ p=0.001 ตามลำดับ
สรุปผล: การผ่าตัดต้อกระจกด้วยเทคนิค Phacoemulsification มีประสิทธิภาพมากกว่า Extracapsular Cataract Extraction และควรพิจารณาการคัดกรองผู้ป่วยเพื่อรับการผ่าตัดต้อกระจกอย่างเหมาะสมเพื่อลดภาวะตาบอดจากโรคต้อกระจก เพื่อการมองเห็นและคุณภาพชีวิตที่ดีขึ้น
