ปัจจัยที่มีความสัมพันธ์กับระดับน้ำตาลในเลือด (HbA1c) ของผู้ป่วยเบาหวานชนิดที่ 2 โรงพยาบาลสมเด็จพระญาณสังวร จังหวัดเชียงราย

บทนำ: โรคเบาหวานเป็นโรคที่มีระดับน้ำตาลในเลือดสูง มากกว่าร้อยละ 90 พบเป็นโรคเบาหวานชนิดที่ 2 และจัดเป็นสาเหตุของการเสียชีวิตอันดับที่ 3 ของโรคไม่ติดต่อเรื้อรัง
วัตถุประสงค์: เพื่อศึกษาปัจจัยที่มีความสัมพันธ์กับระดับน้ำตาลในเลือด (HbA1c) ของผู้ป่วยเบาหวานชนิดที่ 2
ระเบียบวิธีวิจัย: การศึกษานี้เป็นวิจัยแบบ Case – Control Study Design ศึกษาในกลุ่มผู้ป่วยโรคเบาหวานชนิดที่ 2 ที่ขึ้นทะเบียนเข้ารับการรักษาคลินิกโรคไม่ติดต่อเรื้อรัง โรงพยาบาลสมเด็จพระญาณสังวร เก็บข้อมูลช่วงวันที่ 1 ตุลาคม 2565 - 31 มกราคม 2566 อัตราส่วน Case : Control เป็น 1 : 1 กลุ่ม Case คือ ผู้ป่วยที่ค่า HbA1c ≥7 % จำนวน 169 คน กลุ่ม Control คือ ผู้ป่วยที่ค่า HbA1c < 7 % จำนวน 169 คน รวมทั้งหมด 338 คน เก็บข้อมูลจากแบบเก็บข้อมูลโรงพยาบาลและแบบประเมินความรอบรู้ด้านสุขภาพและพฤติกรรมสุขภาพของผู้ป่วย(3อ.2ส.) สถิติที่ใช้ในการวิเคราะห์หาความสัมพันธ์ คือ Multivariable Logistic Regression
ผลการศึกษา: พบปัจจัยที่มีความสัมพันธ์กับระดับน้ำตาลในเลือดของผู้ป่วย ได้แก่ ทักษะด้านการรับประทานยาและมาตรวจตามนัด โดยผู้ป่วยที่มีทักษะในระดับไม่ดี-พอใช้ มีโอกาสควบคุมระดับน้ำตาลในเลือดไม่ได้มากกว่าผู้ป่วยที่มีทักษะอยู่ในระดับดี-ดีมาก 10.03 เท่า (95%CI=1.23-81.70, p=0.031) และ การมีภาวะแทรกซ้อนทางไต โดยผู้ป่วยที่มีภาวะแทรกซ้อนทางไตมีโอกาสควบคุมระดับน้ำตาลในเลือดไม่ได้มากกว่าผู้ป่วยที่ไม่มีภาวะแทรกซ้อนทางไต 1.98 เท่า (95%CI=1.25-3.14, p=0.003)
สรุปผลการวิจัย: ในการศึกษานี้ พบปัจจัยที่มีความสัมพันธ์กับระดับน้ำตาลในเลือด (HbA1c) ของผู้ป่วยเบาหวานชนิดที่ 2 สามารถนำมาใช้ประกอบการตัดสินใจในการรักษาหรือติดตามแนวโน้มระดับน้ำตาลในเลือดของผู้ป่วยในอนาคต รวมถึงการออกแบบแก้ไขปัญหาของผู้ป่วยในพื้นที่ได้อย่างจำเพาะเจาะจงมากยิ่งขึ้น
