การพัฒนาระบบเพื่อสร้างความพึงพอใจและคุณภาพชีวิตในมิติสุขภาพช่องปากสำหรับดูแลผู้ป่วยใส่ฟันเทียมทั้งปากรองรับด้วยรากฟันเทียมเฉลิมพระเกียรติฯระบบข้าวอร่อย โรงพยาบาลกำแพงเพชร จังหวัดกำแพงเพชร

วัตถุประสงค์: เพื่อศึกษาความพึงพอใจและคุณภาพชีวิตในมิติสุขภาพช่องปากในผู้ป่วยใส่ฟันเทียมทั้งปากรองรับด้วยรากฟันเทียมที่มีระบบการตรวจติดตามดูแลรักษาต่อเนื่องแตกต่างกัน นำไปสู่การพัฒนาระบบการตรวจติดตามดูแลรักษาต่อเนื่องที่เหมาะสมสำหรับผู้ป่วยเฉพาะรายให้เกิดประโยชน์สูงสุด
วิธีการศึกษา: เป็นการวิจัยและพัฒนา โดยศึกษาในกลุ่มผู้ป่วยที่ใช้ระบบการตรวจติดตามดูแลรักษาที่พัฒนาร่วมกันระหว่างโรงพยาบาลส่งเสริมสุขภาพตำบลและโรงพยาบาลกำแพงเพชร เปรียบเทียบกับผู้ป่วยที่ได้รับการตรวจติดตามดูแลรักษาแบบปกติที่โรงพยาบาลกำแพงเพชร ระหว่างมีนาคม 2564 ถึงพฤษภาคม 2565 เครื่องมือที่ใช้ในการวิจัยคือ การประเมินสภาพเนื้อเยื่อรอบรากฟันเทียม, แบบสอบถามความพึงพอใจ, แบบสอบถามคุณภาพชีวิตในมิติสุขภาพช่องปากและการสัมภาษณ์เชิงลึก วิเคราะห์และอธิบายข้อมูลด้วยสถิติเชิงพรรณนา ได้แก่ ค่าความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน
ผลการศึกษา: ผู้ป่วยทั้งหมด 30 ราย แบ่งเป็นกลุ่มศึกษา 15 ราย และกลุ่มควบคุม 15 ราย เข้าระบบการตรวจติดตามดูแลรักษาต่อเนื่องที่แตกต่างกันเป็นเวลา 6 เดือน ได้ผลดังนี้ สภาพเนื้อเยื่อรอบรากฟันเทียม ส่วนใหญ่มีภาวะเยื่อเมือกอักเสบรอบรากฟันเทียมโดยไม่มีการสูญเสียกระดูกที่รองรับทั้งสองกลุ่ม ความพึงพอใจอยู่ในระดับมากจนเกือบมากที่สุดโดยค่าเฉลี่ยอยู่ที่ 4.9 (ส่วนเบี่ยงเบนมาตรฐานเท่ากับ 0.1) ในกลุ่มศึกษา และ 4.9 (ส่วนเบี่ยงเบนมาตรฐานเท่ากับ 0.2) ในกลุ่มควบคุม และคุณภาพชีวิตในมิติสุขภาพช่องปากพบว่า การใช้ฟันเทียมทั้งปากรองรับด้วยรากฟันเทียมเฉลิมพระเกียรติฯระบบข้าวอร่อย มีผลกระทบต่อการดำเนินชีวิตน้อยที่สุดโดยค่าเฉลี่ยอยู่ที่ 1.0 (ส่วนเบี่ยงเบนมาตรฐาน เท่ากับ 0) ในกลุ่มศึกษาและ 1.0 (ส่วนเบี่ยงเบนมาตรฐานเท่ากับ 0.1) ในกลุ่มควบคุม
สรุป: ผู้ป่วยทั้งสองกลุ่ม ส่วนใหญ่มีความพึงพอใจอยู่ในระดับมากจนเกือบมากที่สุด รวมทั้งมีคุณภาพชีวิตในมิติสุขภาพช่องปากที่ดีถึงดีมากเช่นกัน โดยเฉพาะเรื่องการเดินทางและความเป็นกันเองของทันตบุคลากรในกลุ่มศึกษา
