Predicting Factors of the Risk of Atrial Fibrillation among People Aged 60 Years and Over in Pattani Province

Abstract

Atrial fibrillation is commonly found in the elderly and is a major factor increasing the risk of stroke, heart failure, and premature death. Pattani province has a continuously increasing trend of morbidity and mortality rates from stroke and heart disease. Therefore, there is a need to develop a screening system and identify predictors of AF risk to inform planning for proactive surveillance in the elderly population. This research aims to 1) study the prevalence of AF in the population aged 60 years and over in Pattani province, and 2) study the predictors of AF in this population group. This is a cross-sectional analytical study from the screening database of people receiving services at all public health service units and communities in Pattani province between March and April 2023. The sample consisted of 10,418 individuals. Data were analyzed using descriptive statistics and multiple logistic regression. The level of statistical significance was set at 0.05. The study results found 139 cases of AF, accounting for 1.33%. The multiple logistic regression analysis revealed that the factors capable of significantly predicting the occurrence of AF included family history of heart disease/arrhythmia/stroke (Adj.OR = 4.63, 95% CI: 3.05–7.04, p < 0.001), systolic blood pressure (Adj.OR = 1.02 per 1 mmHg, 95% CI: 1.01–1.03, p < 0.001), diastolic blood pressure (Adj.OR = 0.98 per 1 mmHg, 95% CI: 0.96–0.99, p = 0.007), and heart rate (Adj.OR = 1.03 per 1 beat/min, 95% CI: 1.01–1.04, p = 0.001). In conclusion, screening the elderly by prioritizing family history of cardiovascular disease, along with blood pressure and pulse rate, can help increase the efficiency of identifying the AF risk group. Recommendations: An AF screening system for the elderly should be continuously developed at the community level by linking care across primary, secondary, and tertiary care units, which may contribute to reducing the long-term occurrence of stroke and heart disease complications.
