A Study of Weight Gain and Metabolic Syndrome in Patients Living with HIV Receiving Antiretroviral Therapy: KOCITAF and TLD Follow-up for 12 Months at Udonthani Hospital

Abstract

This retrospective study examined changes in body weight and metabolic status among 103 adult HIV patients treated with either KOCITAF (n=52) or TLD (n=51). Data were collected from medical records from January 2023 to December 2025, with outcomes measured over a 12-month period and tracked at various time points within each group to monitor the effects of antiretroviral therapy on patients.

The results showed that at month 12, the KOCITAF group had a mean body weight increase of 1.90 ± 4.59 kg (95% CI: 0.66 to 3.15; p = 0.001). Renal function (eGFR) improved (3.59 ± 9.88 mL/min/1.73 m² at 6 months; p = 0.011), and creatinine clearance also improved (4.87 ± 9.24 mL/min; p < 0.001). Renal function decline by CKD stage occurred in 6.7% of the KOCITAF group. In contrast, the TLD group showed a decline in eGFR (11.47 ± 11.08 mL/min/1.73 m²; p < 0.001), with renal function decline occurring in 27.1% of patients. Additionally, an eGFR decline of >10% from baseline was observed in 51.0% of the TLD group compared to 9.6% of the KOCITAF group (p < 0.001). After 12 months, triglyceride levels increased in the KOCITAF group (p = 0.012), and overall dyslipidemia occurred in 96.2% of the KOCITAF group compared to 51.0% in the TLD group (p < 0.001).

KOCITAF was associated with sustained weight gain after treatment initiation, while showing significant benefits for renal function. Dyslipidemia remains a potential complication of both regimens; therefore, close monitoring is needed to guide treatment planning and reduce the risk of further complications.
