Leptospirosis surveillance evaluation in hospitals affiliated with Ministry of Public Health in Kalasin province, fiscal year 2017

Abstract

This study was a cross-sectional study to evaluate qualitative and quantitative of leptospirosis surveillance in hospitals affiliated with Ministry of Public Health in Kalasin province, fiscal year 2017. This study was conducted in 9 hospitals affiliated with Ministry of Public Health by stratified random sampling. We compared all medical records of patients diagnosed as leptospirosis, similar diseases and sample of patients diagnosed as fever of unknown origin with report of leptospirosis under disease surveillance (Report 506).

The study indicated that proportion of identified medical records by disease(s) were leptospirosis 42 cases (100.00%), similar disease 709 cases (100.00%), fever of unknown origin 1,036 cases (8.98% account for 100.00% of sample size calculated). The study found that the sensitivity of surveillance system was 30.56% (95%CI=16.35%-48.11%), positive predictive value was 26.19 % (95%CI=13.86%-42.04%), timeliness of surveillance system was 100% (Median=0.14 day, Range 3 days), data completeness. However, date of onset was invalid. Surveillance system was useful, acceptab, simple, flexible and stable.

This study, revealed that sensitivity and positive predictive value  were low. Timeliness and completeness were excellence. Surveillance system was useful, but number of patients was underreported and can caused misinterpretation. Therefore, leptospirosis surveillance system should be strengthened rapidly.
