Abstract

This research is the action research and aimed to study situation of the health care of home-bound and bed-bound elders, and to development of the health care of home-bound and bed-bound elders in health promotion Ban Dongman.  The sample was home-bound and home-bed elderly participants, caregiver, the elderly volunteer, village health community leaders, members of thombon leaders, no formal leader, and health officer, 58 from purposive sampling. The data were collected from assess the health of the elderly, semi-structured interview, measure satisfaction, elderly holistic health care activity.The statistics for data analysis were frequency, percentage, means, standard deviation,        

           The result: Continuous care model development for disability elders in Bandongmone Health promotion hospital ThombonSingkok has systematic care monitoring and evaluation, health officer and multidisciplinary has linked to the health network Carese Continuous Care Model Development For disability elders in Bandongmone Health Promotion Hospital ThombonSingkok include 3 phase. Phase 1 develop knowledge and skills continuous care. Phase 2 Plan continuous care and practice in community. Phase 3 evaluate after practice and great complacency for Continuous care model development for disability elders
