Factors influence to the treatment results of chronic kidney disease in diabetic patients in Udon Thani province

Abstract

This retrospective analytical study was to study personal characteristic and patient  characteristic factors influence to the treatment results of type 2 diabetes from the hospital in Udon Thani province area by the Purposive sampling were included 256 examples, by questionnaires were employed to collected two set of data; the first in  September, 2013 and second on April, 2014. The analysis approach of the descriptive statistics and inferential statistics were used multiple logistic regression with significant level of p-value = 0.05.

The results indicated that the most examples group were female which age 40 years and over.  BMI good control were increasing 7.3% (from 32.4 to 39.7), overall of chronic kidney disease (CKD) patients not to increase, CKD Stages 4,5 decreasing to CKD Stages 1-3 were 9.4% from 15.9 down to 6.5%, the results treatment at estimated Glomerular Filtration Rate (eGFR) were decreasing from 32.9% up to 93.3%, blood pressure good control were increasing 36.5% (Median 130/74, SD±53.3/30.8) increasing from 51.8 to 88.3%, HbA1C (Median 8.1, SD±2.8) good control were increasing 9.7 increasing from 45.1 to 54.9%, fasting blood sugar(FBS) (Median 138, SD±53.8) good control were increasing 13.4% increasing from 33.4 to 46.8% and found factors were could predicted the treatment results of type 2 diabetes with CKD Stages significantly were as follows: age group lower than 40 years were risk to CKD Stages at 1.79 times (OR 1.79, 95%CI =1.790-1.794, p-value<0.001), FBS at 1.67 times (OR1.67, 95%CI =1.167-2.394, p-value=0.005), body mass index (BMI) at 1.49 times (OR 1.49, 95%CI =1.187-1.893, p-value<0.001), male at 1.46 times (OR 1.46, 95%CI =1.141-1.886, p-value=0.003), blood pressure at 1.08 times (OR 1.08, 95%CI =0.874-1.349, p-value<0.001) and HbA1c at 0.75 times (OR 0.75, 95%CI =0.608-0.937, p-value=0.011). While surveillance of risk factors was found that significant (p<0.001) according to eGFR. Conclusion: These findings illustrate the evaluation of eGFR with initial on special that CKD stage 1-2 could be increase to re-generate of renal function. The recommendations from this finding: the participation of the patient treatment such as diet and BMI could prevent complications of chronic renal failure in chronic kidney diseas.
