การพัฒนาการดูแลผู้ป่วยจิตเวชของเครือข่ายสุขภาพจิตโรงพยาบาลส่งเสริมสุขภาพตำบลคำนาดี อำเภอเมืองบึงกาฬ จังหวัดบึงกาฬ

Abstract

การวิจัยเชิงปฏิบัติการนี้มีวัตถุประสงค์เพื่อศึกษาการพัฒนาการดูแลผู้ป่วยจิตเวชของเครือข่ายสุขภาพจิตโรงพยาบาลส่งเสริมสุขภาพตำบลคำนาดี อำเภอเมืองบึงกาฬ จังหวัดบึงกาฬ กลุ่มเป้าหมายเป็นกลุ่มเครือข่ายสุขภาพจิตจำนวน 56 คนประกอบด้วย ผู้ใหญ่บ้านอาสาสมัครสาธารณสุขประจำหมู่บ้าน ญาติผู้ดูแลผู้ป่วยจิตเวชและกลุ่มผู้ป่วยจิตเวชจำนวน 24 คนกระบวนการศึกษาประกอบด้วย การวิเคราะห์ปัญหา การวางแผนแก้ไขปัญหา การปฏิบัติการและการติดตามประเมินผล ผู้วิจัยเก็บรวบรวมข้อมูลทั้งเชิงปริมาณและคุณภาพด้วยวิธีการตอบแบบสอบถาม การสังเกตและการสัมภาษณ์ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา ได้แก่ ความถี่ ร้อยละ ค่าเฉลี่ยและส่วนเบี่ยงเบนมาตรฐาน สำหรับข้อมูลเชิงคุณภาพใช้การวิเคราะห์เชิงเนื้อหา ผลการวิจัย กระบวนการพัฒนาการดูแลผู้ป่วยจิตเวชของเครือข่ายสุขภาพจิตครั้งนี้ ประกอบด้วย 9 ขั้นตอน ได้แก่ 1) การรวบรวมข้อมูล 2) การวิเคราะห์บริบทพื้นที่  3) การวิเคราะห์ปัญหา 4) การกำหนดแนวทางการแก้ไขปัญหา 5) การจัดทำแผนงาน/โครงการ 6) การบริหารจัดการแผน   7) การปฏิบัติการตามแผน 8) การติดตามและประเมินผล และ 9) การสรุปผล กระบวนการดังกล่าวนำไปสู่แนวทางการพัฒนาการดูแลผู้ป่วยจิตเวชในชุมชนที่เหมาะสมคือ การออกติดตามเยี่ยมผู้ป่วยจิตเวชที่บ้านโดยเครือข่ายสุขภาพจิต การสร้างพลังให้ชุมชนเข้ามารับดำเนินการในปัญหาเอง การพัฒนาทักษะส่วนบุคคลและจัดระบบบริการสาธารณสุขให้ผู้ป่วยจิตเวชเข้าถึงได้สะดวก  โดยปัจจัยความสำเร็จในครั้งนี้ ประกอบด้วย การสร้างเครือข่ายสุขภาพจิตในชุมชน โดยดึงองค์กรปกครองส่วนท้องถิ่นเข้ามามีบทบาทสำคัญ โดยมีโรงพยาบาลส่งเสริมสุขภาพตำบลเป็นผู้ประสานงานและเชื่อมต่องาน ข้อเสนอแนะการพัฒนาการดูแลผู้ป่วยจิตเวชของเครือข่ายสุขภาพจิตในชุมชนต้องอาศัยศักยภาพและความเข้มแข็งของชุมชนเป็นพื้นฐาน โดยมีกระบวนจัดการเพื่อให้หน่วยงานรัฐสนับสนุนงบประมาณและวิชาการโดยยึดหลักการทำงานร่วมกันเป็นภาคีเครือข่ายอย่างดุลยภาพ

The Development of a Health Care Network for Psychiatric Patient in Khamnadee Sub-District, MeuangBuengKan District, BuengKan Province

This action research aimed to find out the care model of psychiatric patients in Khamnadee Sub-District, MeuangBuengKan District, BuengKan Province. The Participants were divided into two groups as 56 community leaders, health volunteers, and caregivers and the 24 psychiatric patients.The study process was consisted with problem analysis, problem solution planning, workshop, and monitoring and evaluation. Data collection included both quantitative and qualitative. It applied methods of questionnaires and interviews.The data was analyzed by descriptive statistics; Frequency, Percentage, Average sands Standards Deviation. Qualitative data was collected by the content analysis.The results of this research found that the development processes of a health caresnetwork sfor psychiatricspatients consisted of 9 steps including: 1) gathering information, 2)context analysis, 3)analysis of the problem, 4) the guidelines formulation,s5 ) the preparation of plans/activities, 6)arranging  plans, 7) implementation  plan, 8) monitoring and evaluation and 9) conclusion. This process was leading to the community care model of appropriate psychiatric care which included home visit activities with the community network, to empowering communities to take action themselves, capacity building the key actors in the community in personal skills and to organize health services to have convenient access for psychiatric patients to have better health care.In conclusion, the success factors included empowering the community network and the local government organization to take action and responsibility while the sub-district health promoting hospital was positioned as the coordinator. Suggestion of this study should be empowering the community network and supportive of all the key actors both public and private sector in the area to come along  to work together as partners where as the main purpose is to support the local network and their partnership to involve activities with managerial effectively.
