Abstract

แนวคิดครอบครัวเป็นศูนย์กลาง เป็นแนวคิดสำคัญที่มีการนำมาใช้อย่างต่อเนื่องทั้งในกลุ่มผู้ป่วยเด็กและผู้ป่วยโรคเรื้อรัง โดยเฉพาะกลุ่มผู้ป่วยที่จำเป็นต้องพึ่งพาการดูแลจากครอบครัว วัตถุประสงค์ของบทความวิชาการนี้ เพื่อถ่ายทอดประสบการณ์ผ่านกรณีตัวอย่างผู้ป่วยเด็กที่จำเป็นต้องพึ่งพาเครื่องช่วยหายใจ โดยการประยุกต์ใช้แนวคิดครอบครัวเป็นศูนย์กลาง แนวคิดนี้ประกอบด้วยการแลกเปลี่ยนข้อมูลร่วมกัน การยอมรับ ให้เกียรติ ระหว่างบุคคล การเป็นหุ้นส่วนและร่วมมือกัน การเจรจา ต่อรอง เพื่อเตรียมครอบครัวในการตัดสินใจนำบุตรกลับบ้าน การเตรียมเครื่องมือ (เช่น เครื่องช่วยหายใจ เครื่องดูดเสมหะ แท็งก์บรรจุออกซิเจน) วัสดุ อุปกรณ์ สภาพแวดล้อม ที่อยู่อาศัย ตลอดจนช่วยให้ครอบครัวได้เรียนรู้ ฝึกปฏิบัติการดูแลจนเกิดชำนาญ การจัดการกับปัญหาทั้งด้านร่างกาย จิตใจ อารมณ์ของผู้ป่วยเด็ก การปรับการดูแลให้เข้ากับวิถีชีวิตและบริบทของครอบครัว ตลอดจนการหาแหล่งช่วยเหลือ การประสานแหล่งชุมชน การติดตามอาการ เพื่อให้ครอบครัวสามารถดูแลบุตรพึ่งเครื่องช่วยหายใจที่บ้านได้อย่างมีคุณภาพชีวิตที่ดี และปลอดภัย

Experience : Pediatric Ventilator Home Care by Family-Centered Care

Family-centered care concept is an important that has been consistently applied in pediatric patients and chronic diseases patients. Especially the patients who need to rely on family care. The purpose of this article is to share experiences through case studies of children who need to ventilator dependent and applying the family-centered care concept. This concept consists of the information sharing, respect and honoring difference, partnership and collaboration and negotiation prepare for a family make decision to take the children back home. The preparation included tool (for example: respirator, suction machine, oxygen tank) materials, environment, home, learning for practice to care until professional practice, to deal with the problems of patients; physical, psychological and emotional and adjusted care to the context of their lives and families. As well as to help family finding source of help, coordination community, follow up signs and symptom. Participation of all this for a family to take care of ventilator dependent children at home is quality of life and safely.
