การพัฒนารูปแบบการสร้างการมีส่วนร่วมของชุมชน ในการดำเนินงานกองทุนหลักประกัน สุขภาพในระดับท้องถิ่นเทศบาลตำบลโนนแดง อำเภอโนนแดง จังหวัดนครราชสีมา

Abstract

กองทุนหลักประกันสุขภาพในระดับท้องถิ่น  มีหลักการที่สำคัญของการมีส่วนร่วมของชุมชน       ต่อระบบหลักประกันสุขภาพ การวิจัยเชิงปฏิบัติการนี้ เป็นการพัฒนารูปแบบการสร้างการมีส่วนร่วมของชุมชนในการดำเนินงานกองทุนหลักประกันสุขภาพในระดับท้องถิ่นของเทศบาลตำบลโนนแดง อำเภอโนนแดง จังหวัดนครราชสีมา กลุ่มเป้าหมายประกอบด้วย คณะกรรมการ อนุกรรมการ และภาคีเครือข่ายกองทุน จำนวน 40 คน ขั้นตอนการวิจัยประยุกต์แนวคิดการวิจัยปฏิบัติการ ประกอบด้วย 1) การวางแผน 2) การปฏิบัติ 3) การสังเกต 4) การสะท้อนผล เครื่องมือที่ใช้ในการเก็บข้อมูลเป็นแบบสอบถาม และแบบบันทึกกิจกรรมตามแผนปฏิบัติการ สถิติที่ใช้ในการวิเคราะห์ข้อมูลคือ ความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และการทดสอบค่าที ผลการวิจัย พบว่า การพัฒนารูปแบบการสร้างการมีส่วนร่วมของชุมชนในการดำเนินงานกองทุนหลักประกันสุขภาพในระดับท้องถิ่นครั้งนี้ มีขั้นตอนคือ 1) การศึกษาบริบท 2) การประชุมปฏิบัติการโดยการใช้แผนที่ทางเดินยุทธศาสตร์ 3) การพัฒนาศักยภาพ  การประชุมประชาคม  การศึกษาดูงาน 4) การติดตามประเมินผลและเปรียบเทียบผลการดำเนินงาน 5) การถอดบทเรียน  ผลการดำเนินงานภายหลังการพัฒนาการ พบว่า มีการเปลี่ยนแปลงดีขึ้นทั้ง 3 ด้าน คือ ความรู้เกี่ยวกับกองทุน การมีส่วนร่วมในกระบวนการบริหารจัดการ การมีส่วนร่วมในการดำเนินงานกองทุนเปลี่ยนแปลงไปอย่างมีนัยสำคัญ และผลการประเมินกองทุน เพิ่มขึ้น ได้ 96 คะแนน อยู่ในระดับเกรด  A ⁺  ปัจจัยแห่งความสำเร็จ คือ 1) การ บูรณาการจากทุกภาคส่วนทั้งองค์กรประชาชนและรัฐ (Integration) 2) การสร้างความรู้สึกรับผิดชอบร่วมกัน (Responsible) 3) ความร่วมมือร่วมใจเป็นอันหนึ่งอันเดียวกัน (Unite) 4) ธรรมาภิบาล (Good governance) ทำให้การดำเนินงานกองทุนประสบความสำเร็จ        มีประสิทธิภาพ ข้อเสนอแนะในการวิจัย กองทุนหลักประกันสุขภาพในระดับท้องถิ่น ควรส่งเสริมการมีส่วนร่วมในการสมทบเงินกองทุนจากประชาชนในพื้นที่ จะทำให้ประชาชนเกิดความเป็นเจ้าของกองทุนและยั่งยืนต่อไป

The Development of  Participation Community Model to Local Health Security Fund Operations at Non Daeng Sub-district Municipality, Non Daeng District, Nakhon Ratchasima Province

The Local Health Security Fund (LHSF). It was the majority principle of involvement community participation to the health security system. This action research was the development  of the community participation model of the local health security fund operations at Non Daeng Sub-district Municipality, Non Daeng District, Nakhon Ratchasima Province. The samples were 40 stakeholders of the executive committee and the partnership network of Non Daeng Sub-district Municipality local health security fund. This study was applied action research concept with 1) planning       2) action 3) observation 4) reflection. Data were collected using questionnaires and recording form in related to the action plan. Data were analyzed by using percentage, mean, standard deviation and paired t-test. The results showed that the involvement community participation of the LHSF operation by used the process of 1) study context 2) participation plan meeting with strategic route map (SRM) 3) meeting to improve knowledge, community meetings and study tour 4) evaluation, follow-up and operation compared 5) lessons distilled. The post development operation was found that, the partnerships were influenced to improve in 3 aspects including, the knowledge of the LHSF, the participation in management and the participation in the operation, were changed of significance and evaluation of the LHSF was increased to 96 points as the grade A⁺. The success factors in LHSF were the integration, the responsible, the unite and the good governance. That was the model of participation network with implementation process. The recommendation for the joint contribution of the local. People are ownership for sustainability and continuity.
