บทบาทพยาบาลชุมชนในการสร้างเสริมความรอบรู้ทางสุขภาพ ในผู้สูงอายุที่เป็นโรคเรื้อรัง

ผู้สูงอายุเป็นกลุ่มเปราะบางที่มีความรอบรู้ทางสุขภาพจำกัดหรือไม่เพียงพอ โดยเฉพาะกลุ่มผู้สูงอายุที่เป็นโรคเรื้อรังต้องเข้ารับบริการในสถานบริการปฐมภูมิอย่างต่อเนื่อง ผู้สูงอายุได้รับข้อมูลที่เกี่ยวข้องกับการดูแลสุขภาพ จากผู้ให้บริการสุขภาพในระบบบริการสุขภาพ และจากการแสวงหาข้อมูลด้วยตนเองจากสื่อสารมวลชน ข้อมูลที่ได้รับมีทั้งข้อมูลที่มีความน่าเชื่อถือและไม่น่าเชื่อถือ ทำให้ผู้สูงอายุจะต้องทำความเข้าใจ เพื่อใช้ในการตัดสินใจดูแลสุขภาพตนเองอย่างถูกต้องและเหมาะสม พยาบาลชุมชนเป็นผู้ที่มีบทบาทในการให้ข้อมูลสุขภาพแก่ผู้สูงอายุที่มารับบริการสุขภาพในสถานบริการปฐมภูมิ จึงมีส่วนสำคัญในการเสริมสร้างความรอบรู้ทางสุขภาพของผู้สูงอายุ บทความวิชาการนี้มีวัตถุประสงค์เพื่ออธิบายความรอบรู้ทางสุขภาพของผู้สูงอายุ วิธีการสร้างเสริมความรอบรู้ทางสุขภาพในผู้สูงอายุ และบทบาทพยาบาลชุมชนในการเสริมสร้างความรอบรู้ทางสุขภาพของผู้สูงอายุ โดยประเมินระดับความรอบรู้ทางสุขภาพ วางแผนและสอนหรือให้ข้อมูลทางสุขภาพที่เหมาะสมกับระดับความรอบรู้ทางสุขภาพของผู้สูงอายุที่เป็นโรคเรื้อรังแต่ละบุคคล เพื่อให้สามารถนำข้อมูลทางสุขภาพ ไปใช้ในการปรับเปลี่ยนพฤติกรรมสุขภาพหรือดูแลสุขภาพของตนเองได้อย่างเหมาะสม
The Roles of Community Nurse for Promoting Health Literacy in the Elderly with Chronic Illness
The elderly as a vulnerable population that are likely to have limit health literacy. Especially, those with chronic illness that are continuously into primary care services. They were received various health information about self-care by health care providers and seek health information from mass medias by themselves. In addition, there are reliable and unreliable of health information which are affected to understand and used health information to decision-making and to apply appropriation of their health. Community nurses play the important roles to provide health information for promoting health literacy of the elderly in primary care. The objectives of this article were to describe health literacy of the elderly, to describe an approach to promote health literacy and to describe the role of community nurse for enhancing health literacy in an elderly person. Community nurses can be assessing health literacy levels, planning and providing appropriately health information for health literacy level of elderly with chronic illness in order to use health information to change their health behaviors or take care of them.
