ผลของโปรแกรมส่งเสริมการดูแลแผลเรื้อรังที่บ้านต่อการหายของแผล ความรู้และพฤติกรรมการดูแลตนเองของผู้ป่วย

บทคัดย่อ
การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง แบบหนึ่งกลุ่มวัดผลก่อนและหลังการทดลอง มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมส่งเสริมการดูแลแผลเรื้อรังที่บ้านต่อการหายของแผล ความรู้และพฤติกรรมการดูแลตนเองของผู้ป่วย กลุ่มตัวอย่างเลือกแบบเฉพาะเจาะจง ได้แก่ ผู้ป่วยแผลเรื้อรังที่อาศัยอยู่ในอำเภอเมืองพิษณุโลก รับการรักษาเครือข่ายสุขภาพโรงพยาบาลพุทธชินราช พิษณุโลก จำนวน 42 คน เครื่องมือที่ใช้ในการศึกษา ได้แก่ 1) แบบสอบถามข้อมูลส่วนบุคคล 2) แบบสอบถามความรู้ ค่าความเชื่อมั่น KR-20 เท่ากับ 0.86 3) แบบสอบถามพฤติกรรมการดูแลตนเองของผู้ป่วยเรื้อรัง ค่าความเชื่อมั่นสัมประสิทธิ์แอลฟาของครอนบาคเท่ากับ 0.89 และ 4) แบบประเมินติดตามการหายของแผลโดยใช้ Pressure Ulcer Scale for Healing (PUSH Tool) Version 3.0 ค่าความเชื่อมั่นเท่ากับ 0.91 และโปรแกรมส่งเสริมการดูแลแผลเรื้อรังที่บ้านตนเอง โดยประยุกต์ใช้ทฤษฎีการดูแลตนเองของโอเร็ม (Orem’s Self-Case Theary) ร่วมกับแนวคิดการจัดการเรียนรู้แบบมีส่วนร่วม (Participatory Learning) วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา Paired t-test และการหายของแผลติดตามผล 3 ครั้ง โดยใช้สถิติ Repeated measures ANOVA ผลการวิจัย พบว่าภายหลังได้รับโปรแกรม กลุ่มตัวอย่างมีคะแนนความรู้หลังการทดลองสูงกว่าก่อนการทดลองอย่างมีนัยสำคัญทางสถิติ (p-value < 0.001) คะแนนพฤติกรรมการดูแลตนเองหลังการทดลองสูงกว่าก่อนการทดลองอย่างมีนัยสำคัญทางสถิติ (p-value = 0.001) และการหายของแผลสูงกว่าก่อนได้รับโปรแกรม อย่างมีนัยสำคัญทางสถิติ (p-value < 0.001) สรุปได้ว่าโปรแกรมการส่งเสริมการดูแลแผลเรื้อรังที่บ้าน ช่วยเพิ่มความรู้ พฤติกรรมการดูแลตนเอง และส่งเสริมการหายของแผล สามารถนำไปประยุกต์ใช้ในการพัฒนารูปแบบการดูแลผู้ป่วยแผลเรื้อรังที่บ้าน
