Effects of Home-Based Chronic Wound Care Promotion Program on Wound Healing, Knowledge and Self-care Behavior Among Patients with Chronic Wounds

Abstract

This study employed a quasi-experimental design using a one-group pretest–posttest approach. The objective was to examine the effects of a home-based chronic wound care promotion program on wound healing, knowledge, and self-care behaviors among patients. The sample was selected using purposive sampling and consisted of 42 patients with chronic wounds residing in Mueang Phitsanulok District, who received care within the health service network of Buddhachinaraj Phitsanulok Hospital. The research instruments included: (1) a demographic questionnaire; (2) a knowledge questionnaire with a reliability coefficient (KR-20) of 0.86; (3) a self-care behavior questionnaire with a Cronbach’s alpha coefficient of 0.89; and (4) a wound healing assessment tool using the Pressure Ulcer Scale for Healing (PUSH Tool), Version 3.0, with a reliability coefficient of 0.91. The intervention program was developed based on Orem’s Self-Care Theory combined with the concept of participatory learning. Data were analyzed using descriptive statistics, paired t-test, and repeated measures ANOVA to assess wound healing outcomes across three follow-up measurements. The results revealed that, after receiving the program, participants demonstrated significantly higher knowledge and self-care behavior scores compared to before the intervention (p < 0.001). In addition, wound healing outcomes significantly improved (p < 0.001). In conclusion, the home-based chronic wound care promotion program effectively enhanced patients’ knowledge, improved self-care behaviors, and promoted wound healing. The program can be applied to further develop models of home-based care for patients with chronic wounds.
