ความต้องการจำเป็นด้านสุขภาพและคุณภาพชีวิตในมิติสุขภาพของผู้ป่วยสูงอายุ ที่มีภาวะหัวใจล้มเหลวเรื้อรัง

บทคัดย่อ
การวิจัยแบบผสมครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาและหาความสัมพันธ์ระหว่างความต้องการจำเป็นด้านสุขภาพ ความรุนแรงของโรค และคุณภาพชีวิตในมิติสุขภาพของผู้ป่วยสูงอายุที่มีภาวะหัวใจล้มเหลวเรื้อรังภายหลังจำหน่ายในจังหวัดราชบุรี จำนวน 100 คน ระยะที่ 1) ศึกษาสถานการณ์ของปัญหาและหาความสัมพันธ์ระหว่างตัวแปรศึกษา โดยใช้แบบสัมภาษณ์ความรุนแรงของโรค ความต้องการจำเป็นด้านสุขภาพ และการรับรู้คุณภาพชีวิตฯ (ค่าความเชื่อมั่น = 0.79, 0.80, 0.88 ตามลำดับ) วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและสัมประสิทธิ์สหสัมพันธ์เพียร์สัน ระยะที่ 2) ศึกษาความต้องการจำเป็นด้านบริการและด้านข้อมูลสุขภาพ ด้วยการสัมภาษณ์รายบุคคลและชุดคำถามปลายเปิดแบบมีโครงสร้าง วิเคราะห์ข้อมูลด้วยการวิเคราะห์เชิงเนื้อหา ผลการวิจัย พบว่า กลุ่มตัวอย่างมีการรับรู้คุณภาพชีวิตโดยรวมระดับดี (M= 3.93, SD= 0.44) ส่วนใหญ่มีความรุนแรงของโรคอยู่ในระดับ 2 (ร้อยละ 82.00) และ ระดับ 1 (ร้อยละ 18.00) ความต้องการจำเป็นด้านบริการสุขภาพระดับน้อย (M= 1.48, SD= 0.39) และด้านข้อมูลสุขภาพระดับปานกลาง (M= 1.76, SD= 0.27) ความรุนแรงของโรคมีความสัมพันธ์เชิงลบกับคุณภาพชีวิตอย่างมีนัยสำคัญทางสถิติ (r= -.407, p< .01) ข้อค้นพบจากผู้ให้ข้อมูลเชิงคุณภาพ ได้แก่ มีความเชื่อมั่นศรัทธาและความพึงพอใจต่อพฤติกรรมบริการแต่ยังต้องการได้รับข้อมูลสุขภาพที่สำคัญต่อการดูแลสุขภาพของตน มีปัญหาเกี่ยวกับการรอคอยพบแพทย์และสถานที่รอรับบริการไม่สะดวกสบาย จึงต้องการบริการเฉพาะที่ตอบสนองความต้องการของผู้ป่วยสูงอายุ ข้อเสนอแนะ พยาบาลควรตระหนักถึงความสำคัญในการประเมินความต้องการจำเป็นด้านสุขภาพเพื่อเป็นแนวทางจัดบริการสุขภาพที่มีคุณภาพ โดยเน้นการให้ข้อมูลสุขภาพ ได้แก่ โรคและการดำเนินของโรค ผลการตรวจทางห้องปฏิบัติการ การประเมินอาการผิดปกติ การใช้ยา และการฟื้นฟูสมรรถภาพหัวใจ
