บทคัดย่อ
วัตถุประสงค์: การศึกษาเชิงพรรณนาแบบตัดขวางนี้มีวัตถุประสงค์เพื่อศึกษารูปแบบครัวเรือน บทบาทผู้สูงอายุ และเปรียบเทียบความแตกต่างของสภาวะช่องปากและการใช้บริการทันตกรรมของผู้สูงอายุในแต่ละรูปแบบครัวเรือนในผู้ที่มีอายุตั้งแต่ 60 ปีขึ้นไป จำนวน 354 คน อำเภอเวียงหนองล่อง จังหวัดลำพูน
วัสดุอุปกรณ์และวิธีการ: สุ่มตัวอย่างอย่างเป็นระบบและตามสัดส่วนประชากรผู้สูงอายุในแต่ละตำบล เก็บข้อมูลด้วยแบบสอบถามที่ผู้วิจัยสร้างขึ้น ได้แก่ 1) รูปแบบครัวเรือน พิจารณาจากลักษณะสมาชิกในครัวเรือนที่ผู้สูงอายุอาศัย 2) บทบาทผู้สูงอายุ กำหนดจากกิจกรรมที่ผู้สูงอายุใช้เวลาส่วนใหญ่ทำในหนึ่งสัปดาห์ใน 6 เดือนที่ผ่านมา 3) จำนวนฟันผุ อุด และถอน สภาวะปริทันต์ จำนวนฟันในปากและจำนวนฟันคู่สบ 4) การใช้บริการทันตกรรมใน 12 เดือนที่ผ่านมา ตรวจช่องปากโดยทันตาภิบาล 3 คนที่ผ่านการปรับมาตรฐานกับทันตแพทย์และเทียบกันเอง ได้ค่าแคปปา 0.82
ผล: กลุ่มตัวอย่างมีอายุเฉลี่ย 68.63±5.43 ปี ร้อยละ 57.9 เป็นเพศหญิง มีครัวเรือนผู้สูงอายุอยู่กับวัยทำงานร้อยละ 24.5 ครัวเรือนผู้สูงอายุอยู่เพียงลำพังคนเดียว ร้อยละ 18.7 ครัวเรือนเฉพาะผู้สูงอายุร้อยละ 28.6 และครัวเรือนผู้สูงอายุอยู่กับเด็กร้อยละ 28.2 ในครัวเรือนผู้สูงอายุอยู่กับเด็ก ร้อยละ 44.4 ผู้สูงอายุมีบทบาทผู้ดูแล ร้อยละ 71.9 มีค่าเฉลี่ยฟันผุถอนอุด 14.12±8.04 ซี่ต่อคน และโรคปริทันต์อักเสบ ร้อยละ 32.3 ซึ่งสูงกว่าผู้สูงอายุที่อาศัยในครัวเรือนรูปแบบอื่นอย่างมีนัยสำคัญทางสถิติ (p<0.001) อีกทั้งมีการใช้บริการทันตกรรมน้อยที่สุด (ร้อยละ 15.0)
บทสรุป: การศึกษานี้พบความแตกต่างของสภาวะโรคในช่องปากและการใช้บริการทันตกรรมของผู้สูงอายุในแต่ละรูปแบบครัวเรือน ดังนั้นปัจจัยดังกล่าวควรถูกนำมาพิจารณาในการออกแบบบริการเพื่อเพิ่มการเข้าถึงและการใช้บริการทันตกรรมต่อไป
