บทคัดย่อ
วัตถุประสงค์: ผลิตภาพเป็นการวัดความสามารถในการทำงาน โดยเปรียบเทียบผลลัพธ์ที่ได้กับระยะเวลาหรือต้นทุนที่ใช้ ซึ่งปัจจุบันระบบบริการสุขภาพช่องปากยังขาดวิธีประเมินการทำงานของทันตบุคลากรให้เหมาะสมกับบริบททางทันตกรรมที่มีหัตถการที่หลากหลาย การศึกษานี้มีวัตถุประสงค์เพื่อประเมินผลิตภาพการให้บริการทันตกรรมของทันตบุคลากรในเครือข่ายหน่วยบริการสุขภาพ อำเภอกระสัง จังหวัดบุรีรัมย์ ภายใต้ระบบบริการสุขภาพช่องปากที่มีการพัฒนาด้วยการหมุนเวียนทันตแพทย์จากโรงพยาบาลออกให้บริการงานทันตกรรมเฉพาะทางที่ รพ.สต. เป้าหมาย
วัสดุอุปกรณ์และวิธีการ: เก็บข้อมูลเปรียบเทียบช่วงก่อนและหลังการพัฒนาระบบบริการ ในเดือนกรกฎาคม 2562 – มิถุนายน 2563 และกรกฎาคม 2565 – มิถุนายน 2566 โดยคำนวณผลิตภาพจากผลรวมของบริการทันตกรรมแต่ละกิจกรรมที่ทำได้นำมาคูณเพื่อถ่วงน้ำหนักด้วยเวลามาตรฐานที่อ้างอิงจากคู่มือการจ่ายค่าตอบแทนตามผลการปฏิบัติงาน (P4P) แล้วหารด้วยเวลาให้บริการจริงของทันตบุคลากร แยกตามสถานบริการ ได้แก่ 1) โรงพยาบาล 2) รพ.สต. เป้าหมายที่
ทันตแพทย์ออกให้บริการ และ 3) รพ.สต. อื่นในเครือข่าย
ผล: ผลิตภาพการให้บริการทันตกรรม แบ่งออกเป็นผลิตภาพงานเฉพาะทางและผลิตภาพงานทั่วไปและส่งเสริมป้องกัน พบว่าใน รพ.สต. เป้าหมายสามารถสร้าง
ผลิตภาพงานเฉพาะทาง (0.23) และมีผลิตภาพงานเฉพาะทางในโรงพยาบาลที่เพิ่มขึ้น ร้อยละ 11.63 ขณะที่ผลิตภาพงานทั่วไปและส่งเสริมป้องกันเพิ่มขึ้นในทุกกลุ่มสถานบริการ ได้แก่ โรงพยาบาล (ร้อยละ 34.48) รพ.สต. เป้าหมาย (ร้อยละ 12.63) และ รพ.สต. อื่น (ร้อยละ 3.33)
บทสรุป: การประเมินผลิตภาพการให้บริการทันตกรรมควรแยกตามลักษณะของการให้บริการเพื่อให้สามารถพัฒนาการให้บริการได้ตรงกับการทำงานจริง ซึ่งระบบบริการสุขภาพช่องปากของอำเภอกระสัง จังหวัดบุรีรัมย์ สามารถเพิ่มผลิตภาพและพัฒนาการให้บริการทันตกรรมได้ทั้งเครือข่ายหน่วยบริการสุขภาพ อันเป็นแนวทาง
ต่อยอดในการพัฒนาระบบบริการสุขภาพช่องปากต่อไป
