การวิเคราะห์การใช้บริการทันตกรรมของประชาชนไทยกับนโยบายทันตสาธารณสุขในช่วงทศวรรษที่ผ่านมา

บทคัดย่อ
บทความนี้มีวัตถุประสงค์เพื่อ รวบรวมและวิเคราะห์นโยบายด้านทันตสาธารณสุข และวิเคราะห์การใช้บริการทันตกรรมของ ประชาชนไทยในรอบ 10 ปี โดยใช้ข้อมูลการสำรวจอนามัยและสวัสดิการ ระหว่างปี พ.ศ. 2548 ถึง พ.ศ. 2558 ของสำนักงานสถิติแห่งชาติ วิเคราะห์ข้อมูลการใช้บริการทันตกรรม จำแนกตามเพศ ภูมิภาค กลุ่มอายุ ประเภทการคุ้มครองสุขภาพ สถานบริการและชนิดบริการ จากการใช้บริการครั้งล่าสุดเท่านั้น และ ทบทวนวรรณกรรมด้านนโยบายที่เกี่ยวข้องกับงานทันตกรรมต่างๆที่ผ่านมา ในช่วงเวลาเดียวกัน พบว่า โดยภาพรวม การใช้บริการทันตกรรมของประชาชนไทยในช่วง 10 ปี ไม่เปลี่ยนแปลงมากนัก (ระหว่างร้อยละ 8.11-9.47) เพศหญิง มีอัตราการใช้บริการมากกว่าเพศชาย ทุกการสำรวจ กลุ่มอายุที่มีการใช้บริการมาก คือ กลุ่มอายุ 5-14 ปี อย่างไรก็ดี กลุ่มเด็ก 0-4 ปีมีอัตรา การใช้บริการทันตกรรมเพิ่มขึ้นอย่างต่อเนื่อง คลินิกและโรงพยาบาลเอกชนมีบทบาทสำคัญในการให้บริการร้อยละ 35-44 สอดคล้องกับ การกระจายตัวของทันตบุคลากร ประชาชนเขตกรุงเทพฯมีอัตราการใช้บริการทันตกรรมมากที่สุด ซึ่งเมื่อเปรียบเทียบกับการใช้บริการ ของประชาชนในภาคตะวันออกเฉียงเหนือ (ภูมิภาคที่มีการใช้บริการต่ำที่สุด) พบว่า มากกว่าประมาณ 1.6-2.0 เท่า ด้วยแนวโน้มความ แตกต่างของการใช้บริการที่เพิ่มขึ้น นโยบายด้านทันตสาธารณสุขในรอบ 10 ปีที่ผ่านมา จึงทำให้เกิดการเพิ่มจำนวนและการกระจายตัวของ บุคลากร เพิ่มศักยภาพการบริการทันตกรรมระดับปฐมภูมิ และเพิ่มการอัตราการใช้บริการทันตกรรมของกลุ่มเป้าหมาย การดำเนินนโยบาย ด้านทันตสาธารณสุขมีบทบาทสำคัญที่ส่งผลต่ออัตราการใช้บริการทันตกรรมของประชาชนไทย แม้ว่าอัตราการใช้บริการในภาพรวมจะยังไม่ ค่อยเปลี่ยนแปลงนัก แต่มีทิศทางที่ดีขึ้นทั้งการใช้บริการในกลุ่มเป้าหมายที่เพิ่มขึ้นและศักยภาพบริการที่เพิ่มขึ้น อย่างไรก็ดี ประเทศไทย ยังจำเป็นต้องติดตามความก้าวหน้าของการดำเนินการและแก้ไขความเหลื่อมล้ำของการใช้บริการทันตกรรมโดยเฉพาะอย่างยิ่งระหว่างภูมิ ภาค ที่ยังมีปัญหาการกระจายบุคลากร
