Abstract

การคลอดบุตรเป็นประสบการณ์หนึ่งที่มีความสำคัญสำหรับสตรีตั้งครรภ์และครอบครัว หากการคลอดบุตรมีระยะเวลายาวนานย่อมจะส่งผลกระทบต่อร่างกายและจิตใจของผู้คลอด นอกจากนี้ผู้คลอดในครรภ์แรกหรือในผู้คลอดครรภ์หลังต่างก็มีระยะเวลาในการคลอดที่แตกต่างกัน โดยในผู้คลอดครรภ์แรกมักมีระยะเวลาในการคลอดนานกว่าผู้คลอดครรภ์หลัง ดังนั้นพยาบาลผดุงครรภ์จึงมีบทบาทสำคัญในการช่วยส่งเสริมความก้าวหน้าของการคลอด ย่นระยะเวลาของการคลอดให้สิ้นสุดลงอย่างมีคุณภาพและมีความปลอดภัย ซึ่งจะรวมถึงการประเมินความก้าวหน้าของการคลอด การหาปัจจัยที่มีผลต่อการดำเนินการคลอด และการวางแผนการพยาบาลให้เหมาะสมตลอดกระบวนการคลอด ได้แก่ การส่งเสริมให้ทารกในครรภ์มีการเคลื่อนต่ำพร้อมกับส่งเสริมให้มดลูกมีการหดรัดตัวที่ดีจะช่วยให้เกิดความก้าวหน้าของการคลอด และลดภาวะแทรกซ้อนหรือผลเสียที่อาจเกิดขึ้นกับมารดาและทารกแรกเกิดลงได้

Progression of Labour : The Midwifery Roles

Giving birth is an important experience for pregnancy women and their families. During a prolonged labour, this could impact on both physical and psychological status. Moreover, the pregnant with nulliparous or multiparous have their own stages of labour. The former takes a longer duration of each stage than those of the latter. Therefore, most midwives have to carry on the major role play promoting the progressing of shorten the first stage of labour with a good quality and safety. These include: evaluating of progression of labour, finding factors that affect to labour process, and planning suitable nursing cares throughout the ongoing process. For example, the promotion of fetal descent passing along with a good uterine contraction will enhance the progression of labour and reduce some complications or poor outcomes that may occur to mother as well as newborn.
