การปฏิบัติบทบาทของพยาบาลวิชาชีพในการสร้างเสริม สุขภาพในเขตพื้นที่ภาคกลาง; Role Performance of Professional Nurses in Health Promotion in Central Region

Abstract

การวิจัยเชิงพรรณนาครั้งนี้ มีวัตถุประสงค์เพื่ออธิบายและเปรียบเทียบการปฏิบัติบทบาทของพยาบาลวิชาชีพ ในการ สร้างเสริมสุขภาพตามกรอบกฎบัตรออตตาวา (Ottawa Charter) ในการให้บริการสุขภาพระดับปฐมภูมิ ทุติยภูมิ และตติยภูมิ กลุ่มตัวอย่างเป็นพยาบาลวิชาชีพ จำนวน 4,089 คน ที่ปฏิบัติงานในเขตพื้นที่ภาคกลาง 12 จังหวัด ได้แก่ กรุงเทพมหานคร ชัยนาท นครปฐม นนทบุรี ปทุมธานี พระนครศรีอยุธยา สมุทรปราการ สมุทรสงคราม สมุทรสาคร สิงห์บุรี สุพรรณบุรี และอ่างทอง ใช้การสุ่มเลือกกลุ่มตัวอย่างแบบเป็นขั้นตอน เก็บรวบรวมข้อมูลด้วยแบบสอบถามข้อมูลทั่วไป และแบบสอบถามการปฏิบัติบทบาท ในการสร้างเสริมสุขภาพ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และ Welch’s ANOVA ผลการวิจัยพบว่า กลุ่มตัวอย่างส่วนใหญ่ (ร้อยละ 87.8) ซึ่งเป็นพยาบาลวิชาชีพประจำหน่วยบริการ หรือพยาบาลผู้ปฏิบัติการ ขั้นสูง (APN) ในระดับปฐมภูมิ ทุติยภูมิ และตติยภูมิ มีการปฏิบัติบทบาทในการสร้างเสริมสุขภาพตามกรอบกฎบัตรออตตาวา ค่อนข้างจำกัด พยาบาลวิชาชีพที่ให้บริการสุขภาพในระดับปฐมภูมิ มีคะแนนรวมเฉลี่ย และคะแนนเฉลี่ยการปฏิบัติบทบาทในการ สร้างเสริมสุขภาพทุกด้าน สูงกว่าระดับทุติยภูมิและระดับตติยภูมิอย่างมีนัยสำคัญทางสถิติ (p < .01) พยาบาลวิชาชีพที่ให้บริการ สุขภาพในระดับทุติยภูมิ มีคะแนนเฉลี่ยการปฏิบัติบทบาทในการสร้างเสริมสุขภาพ ด้านการเสริมสร้างความเข้มแข็งให้กับชุมชน และการปรับระบบบริการสุขภาพ สูงกว่าระดับตติยภูมิอย่างมีนัยสำคัญทางสถิติ (p < .01) ดังนั้นจึงมีข้อเสนอแนะให้พัฒนาสมรรถนะ ด้านการสร้างเสริมสุขภาพของพยาบาลวิชาชีพ เพื่อช่วยให้สามารถบูรณาการการสร้างเสริมสุขภาพตามกรอบกฎบัตรออตตาวา ในการปฏิบัติงานได้ และควรมีการศึกษาวิจัยต่อไปถึงปัจจัยที่เกื้อหนุนให้พยาบาลวิชาชีพสามารถปฏิบัติบทบาทในการสร้างเสริม สุขภาพตามกรอบกฎบัตรออตตาวา

The purposes of this descriptive study were to describe and compare role performance of professional nurses in health promotion according to Ottawa Charter in primary, secondary, and tertiary care services. The sample was 4,089 professional nurses who have been working in 12 provinces in central region, which included Bangkok Metropolis, Chai Nat, Nakhon Pathom, Nonthaburi, Pathum Thani, Phra Nakhon Si Ayutthaya, Samut Prakan, Samut Songkhram, Samut Sakhon, Sing Buri, Suphan Buri, and Ang Thong. Stratified random sampling was used. Data were collected by Demographic Data Questionnaire and Role Performance in Health Promotion Questionnaire. Data were analyzed by descriptive statistics and Welch’s ANOVA. The findings revealed that the majority of subjects (87.8%) who were professional nurses or advanced practice nurses working at primary, secondary, or tertiary care levels had limited role performance in health promotion according to Ottawa Charter. Professional nurses who provided primary care service had higher overall mean score and all subscale mean scores of role performance in health promotion than those working at secondary and tertiary care levels with statistical significance (p < .01). Professional nurses who provided secondary care service had higher subscale mean scores on “Strengthen community action” and “Reorient health services” than those working at tertiary care level with statistical significance (p < .01). It was suggested that professional nurses’ competency in health promotion should be enhanced so that they will be able to incorporate health promotion according to Ottawa Charter in their practices. Moreover, further studies should be conducted to examine supportive factors for nurses’ health promotion practices according to Ottawa Charter.
