การพัฒนาระบบการนิเทศเพื่อความเป็นเลิศทางการพยาบาลในโรงพยาบาล ค่ายสรรพสิทธิประสงค์ อำเภอวารินชำราบ จังหวัดอุบลราชธานี The Development System for Excellence in Nursing Supervision at Fort Sunpasitthiprasong Hospital Warinchamrab District, Ubon Ratchath

Abstract

บทคัดย่อ

การศึกษาวิจัยครั้งนี้มีความมุ่งหมายเพื่อพัฒนาระบบการนิเทศเพื่อความเป็นเลิศทางการพยาบาลของโรงพยาบาลค่ายสรรพสิทธิประสงค์ ทำการศึกษากับพยาบาลหัวหน้าหอผู้ป่วย 11 คน และพยาบาลประจำการจำนวน 34 คน รวม 45 คน รูปแบบการวิจัยหนึ่งกลุ่มวัดผลก่อนและหลังดำเนินการ ระยะเวลาทำการศึกษา 32 สัปดาห์ โดยการประยุกต์ใช้ทฤษฎีระบบ แนวคิดการจัดการคุณภาพทั่วทั้งองค์กรและแนวคิดการนิเทศการพยาบาลแบบมีส่วนร่วม โดยใช้กิจกรรมประชุมวิชาการ การประชุมเชิงปฏิบัติการ การประชุมกลุ่มสนทนา เพื่อทบทวนนโยบายและกำหนดแผนการปฏิบัติ การติดตามประเมินผล และการสะท้อนผลลัพธ์จากการดำเนินการพัฒนา เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามที่มีค่าความเชื่อมั่นเท่ากับ 0.85 ทำการวิเคราะห์ข้อมูลเชิงปริมาณโดยใช้โปรแกรมคอมพิวเตอร์สำเร็จรูป และข้อมูลเชิงคุณภาพทำการวิเคราะห์โดยการสรุปประเด็นจากการใช้กระบวนการกลุ่ม

สถิติที่ใช้ในการวิจัยคือ สถิติเชิงพรรณนา ได้แก่ การแจกแจงความถี่ ร้อยละ ค่าเฉลี่ยและส่วนเบี่ยงเบนมาตรฐาน สถิติเชิงอนุมานใช้การเปรียบเทียบค่าเฉลี่ย ได้แก่ Paired Sample t-test และ Wilcoxon Signed Rank test

ผลการวิจัยพบว่า หลังดำเนินการพัฒนาระบบการนิเทศการพยาบาล ระดับความรู้และการมีส่วนร่วมของหัวหน้าหอผู้ป่วยและพยาบาลประจำการสูงกว่าก่อนพัฒนาระบบ พฤติกรรมการปฏิบัติตามแผนการนิเทศของหัวหน้าหอผู้ป่วย และความพึงพอใจต่อระบบการนิเทศของพยาบาลประจำการสูงกว่าก่อนพัฒนาระบบ เมื่อเปรียบเทียบค่าทางสถิติพบว่าค่าเฉลี่ยความรู้ พฤติกรรมการปฏิบัติตามแผนการนิเทศ การมีส่วนร่วมและความพึงพอใจ เพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05

โดยสรุปปัจจัยที่มีผลต่อการพัฒนาระบบการนิเทศเพื่อความเป็นเลิศทางการพยาบาลคือ การกำหนดนโยบายที่ชัดเจน วัฒนธรรมองค์กร การมีส่วนร่วม สมรรถนะของบุคลากร การติดตามประเมินผลอย่างต่อเนื่องและให้ข้อมูลสะท้อนกลับไปยังผู้เกี่ยวข้องเพื่อให้เกิดคุณภาพมาตรฐานการพยาบาลทั่วทั้งองค์กรและมีการพัฒนาคุณภาพอย่างต่อเนื่อง

ABSTRACT

This study aimed to development system for excellence in nursing supervision at Fort Sunpasitthiprasong Hospital. This study was conducted 11 Head Nurses and 34 Register nurses a total of 45 person. This study design was measured one group pretest-posttest design. The study period of 32 weeks. There was usually applied system theory, concept participation nursing supervision, total quality management. Using conference activity, workshop and focus group discussion to review the policies and defined the action plan to monitoring, evaluation and reflection of the development. Data were collected using a questionnaire with a reliability of 0.85. Quantitative data were analyzed using computer software packages. And qualitative data were analyzed by the conclusions of the group process.

The statistics used in this study was descriptive statistics including frequency, percentage, mean and standard deviation. Inferential statistics were used to compare mean Paired Sample t-test and Wilcoxon Signed Rank test.

The results showed that after improving supervision system the knowledge and participated of Head nurse higher than the previous system, the behavior of the proposed of Head nurse, satisfaction with the supervision of Registered nurse higher than the previous system. When comparing statistics showed that the average knowledge the behavior of the proposed, participated and satisfaction with the supervision and statistically increased significantly at 0.05 (p-value < 0.001).

In summary, the factors affecting the development of the supervisory system for excellence in nursing was the policy was clearly, organizational culture, the participation, performance of the personnel, continuous evaluation and feed back to stakeholders to achieve quality of standard nursing care and continuous quality improvement
