Abstract

การวิจัยครั้งนี้ เป็นการวิจัยเชิงคุณภาพ มีวัตถุประสงค์เพื่อศึกษาประสบการณ์การแสวงหาความช่วยเหลือของผู้ป่วยวัณโรคปอดรายใหม่ ก่อนเข้ารับการตรวจรักษาที่คลินิกวัณโรค โรงพยาบาลสมุทรปราการ ผู้ให้ข้อมูลคือผู้ป่วยวัณโรคปอดรายใหม่ 14 ราย เก็บรวบรวมข้อมูลโดยการสัมภาษณ์เชิงลึกและการบันทึกเทป และวิเคราะห์ข้อมูลตามวิธีของ Colaizzi (1978) ผลการศึกษาพบว่า ผู้ป่วยวัณโรคมีการแสวงหาความช่วยเหลือก่อนมารับการตรวจรักษาที่คลินิก แบ่งเป็น 3 ระยะ ประกอบด้วย 1) ระยะมีอาการเจ็บป่วยธรรมดา คล้ายโรคหวัด ผู้ป่วยดูแลตนเองและแสวงหาความช่วยเหลือเพื่อรักษาและบรรเทาอาการ 2) ระยะมีอาการเรื้อรังไม่หาย ผู้ป่วยส่วนหนึ่งยังคงดูแลตนเองและแสวงหาความช่วยเหลือเพื่อรักษาและบรรเทาอาการ และอีกส่วนหนึ่งตัดสินใจเข้ารับการตรวจรักษาที่โรงพยาบาล และ 3) ระยะมีอาการรุนแรงน่ากลัว ผู้ป่วยในระยะนี้ทั้งหมดตัดสินใจไปโรงพยาบาลเพื่อรับการรักษาที่เหมาะสม เงื่อนไขที่สนับสนุนให้ผู้ป่วยไปรับการรักษาที่โรงพยาบาล คือ ลักษณะอาการที่เรื้อรังและ/หรือมีความรุนแรง
ความรู้เกี่ยวกับวัณโรค แรงสนับสนุนจากบุคคลรอบข้าง สิทธิการรักษา และระยะทางจากบ้านถึงสถานบริการ ส่วนการรับรู้ว่าเป็นโรคไม่รุนแรง กลัวว่าจะเป็นโรคร้าย ng: normal; line-height: normal; orphaหรือกลัวการไปโรงพยาบาล ค่าใช้จ่ายในการรักษาหรือไม่มีสิทธิการรักษา เป็นเงื่อนไขที่มีผลต่อความล่าช้าในการไปรับการรักษาที่คลินิกของผู้ป่วย

This Qualitative research aim to study explore help-seeking experience of newly pulmonary tuberculosis patients before attending to tuberculosis clinic, Samutprakan Hospital. Participants were 14 newly pulmonary tuberculosis patients. Data were collected through in-depth interview and tape recorded, and analyzed by applying Colaizzi’s method (1978). Findings revealed that tuberculosis patients sought helps and treatments before attending to hospitals, which consisted of 3 phrases including 1) Having common symptoms and illness like common cold. Patients did self-care and had help seeking for treating and alleviating the symptoms 2) Having chronic symptoms. Some patients remained to do self-care and to have help seeking for treating and alleviating the symptoms, and some decided to attend hospital for investigation and advanced treatment. And 3) Having severe and aggressive symptoms. All patients in this stage decided to attend hospitals for appropriate treatment. Conditions that affected to attend hospitals were perception of chronic and/or severe symptoms, knowledge about tuberculosis, social support, type of health insurance, and distance between home and hospital. Perception of having common symptoms, feeling of fear to have terrible illness or attending hospitals, and treatment expenditure/or a lack of health insurance were factors affecting patients to delay in attending tuberculosis clinic.
