บทคัดย่อ

การบริหารความเสี่ยงเป็นเครื่องมือที่สำคัญในการกำกับติดตามในระบบมาตรฐานคุณภาพ และได้มีการนำมาประยุกต์ใช้ในการพัฒนามาตรฐานงานบริการผู้ป่วยในเพื่อการให้บริการเป็นไปอย่างมีคุณภาพ  การวิจัยเชิงปฏิบัติการแบบมีส่วนร่วมครั้งนี้ มีวัตถุประสงค์เพื่อพัฒนาระบบการจัดการความเสี่ยงด้านคลินิกตามมาตรฐานงานบริการผู้ป่วยในของพยาบาลโรงพยาบาลค่ายสรรพสิทธิประสงค์ โดยประยุกต์แนวคิดการพัฒนาคุณภาพและการจัดการความเสี่ยงเพื่อให้เกิดการพัฒนาระบบงานให้ดียิ่งขึ้น ผู้เข้าร่วมกระบวนการพัฒนาได้แก่ พยาบาลประจำหอผู้ป่วยใน จำนวน 32 คน ดำเนินกระบวนการตามขั้นตอนการพัฒนาคุณภาพคือการวางแผน การดำเนินการ การตรวจสอบ และการสะท้อนผล (P-D-C-A)  เก็บรวบรวมข้อมูลโดย แบบสอบถามที่สร้างขึ้น วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนาได้แก่ ค่าเฉลี่ย ค่าเบี่ยงเบนมาตรฐานและสถิติเชิงอนุมานได้แก่ pair t - test

ผลการวิจัยพบว่า หลังดำเนินการพัฒนา ผู้เข้าร่วมกระบวนการมีค่าเฉลี่ยคะแนนความรู้ การปฏิบัติ  การมีส่วนร่วม และความพึงพอใจ ในกระบวนการบริหารความเสี่ยงด้านคลินิกของพยาบาลประจำการในหอผู้ป่วยใน เพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (p-value < 0.001) พบว่า ปัจจัยที่นำไปสู่ความสำเร็จในการดำเนินงานคือ ภาวะผู้นำของผู้นำสูงสุดขององค์กร และการกำหนดบทบาทให้กับทีมผู้พิทักษ์ความเสี่ยง (Protective Risk Management Team-PRMT) โดยการมอบอำนาจการตัดสินใจแก่หัวหน้าหน่วยงาน และสร้างการมีส่วนร่วมของบุคลากรในทีมสุขภาพในทุกระดับ

โดยสรุป กระบวนการพัฒนาคุณภาพในงานบริการด้านการพยาบาล สามารถประยุกต์ใช้การจัดการความเสี่ยงเป็นเครื่องมือกำกับติดตามที่สำคัญ และจำเป็นต้องมีองค์ประกอบอื่นที่จำเป็นเช่น การมีภาวะผู้นำที่ดีของผู้นำสูงสุด การจัดการคุณภาพโดยกำหนดบทบาททีมสนับสนุนที่ชัดเจนและการมอบอำนาจที่เหมาะสม รวมทั้งการสร้างการมีส่วนร่วมในทุกขั้นตอนของผู้มีส่วนได้เสียในกระบวนการพัฒนาคุณภาพ

Abstract

Risk management is an important technique for the quality standard monitoring. There was usually applied to quality development in nursing-care standard system. This participatory action research aimed to develop risk management standards for clinical services improvement process in the Fort Sunprasittiprasong hospital. Applying the concept of quality and risk management into the system development was approached in the process. Participants were a group of 32 nurses in the in-patient department and using a step of the PDCA activities to develop process. Created questionnaires were used for data collecting. Data were analyzed by descriptive statistics such as the average, standard deviation and statistical inference based on pair t - test.

The results show that after development process, participants, the average score on the knowledge, practice, participation and  satisfaction of the process of risk management, were increased significantly (p-value <0.001). The key success factors were included as the leadership of the highest level in the organization, and to assign roles to the team of risk protection (Protective Risk Management Team-PRMT) by authorizing the agency to decide, and the involvement of health personnel in the team at all levels.

In summary, the process of quality improvement in nursing services it is significantly to application of risk management is an important monitoring tool. And other necessary components as required are comprised as the leadership of the highest level and activity of the ac hoc team. Additionally, it is very important to deploy the policy into the role of quality management team, supported by clear and appropriate authority. As well as the participation in all stages of development of stakeholders in the process is an essentially element.
