การจัดอัตรากำลังบุคลากรทางการพยาบาล ตามความต้องการการพยาบาลของผู้ป่วยใน : โรงพยาบาลในสังกัดกรมแพทย์ทหารเรือ Staffing of Nursing Personnel Based On Nursing Care Needs in In-Patient Unit : hospital under the Jurisdiction of the Naval Medical Departmen

Abstract

บทคัดย่อ

การวิจัยครั้งนี้ มีวัตถุประสงค์เพื่อพัฒนาระบบการจำแนกประเภทผู้ป่วย และศึกษาอัตรากำลังบุคลากรทางการพยาบาลตามความต้องการการพยาบาลของผู้ป่วยใน ในโรงพยาบาลสังกัดกรมแพทย์ทหารเรือ กลุ่มตัวอย่างคือ ผู้ป่วยที่เข้ารับการรักษาในหอผู้ป่วย แผนกอายุรเวชกรรม ศัลยกรรม กุมาร-           เวชกรรม และสูติ-นรีเวชกรรม ในโรงพยาบาลสมเด็จพระปิ่นเกล้า และโรงพยาบาลสมเด็จพระนางเจ้าสิริกิติ์ และบุคลากรทางการพยาบาลที่ปฏิบัติงาน จำนวนหอผู้ป่วยละ 30 เวร เก็บรวบรวมข้อมูลโดยใช้แบบจำแนกประเภทผู้ป่วย 5 ประเภท และแบบบันทึกกิจกรรมการพยาบาล สังเกตและบันทึกเวลาที่ใช้ในการปฏิบัติกิจกรรมการพยาบาล เครื่องมือผ่านการตรวจสอบความตรงตามเนื้อหาและมีค่าความเที่ยงระหว่าง .80-1.00 เก็บรวมรวมข้อมูลตั้งแต่เดือน กันยายน - พฤศจิกายน 2552

ผลการวิจัย

1. ได้ระบบการจำแนกประเภทผู้ป่วยของผู้ป่วยใน แผนกอายุรเวชกรรม ศัลยกรรม กุมารเวชกรรม และสูติ-นรีเวชกรรม ของโรงพยาบาลในสังกัดกรมแพทย์ทหารเรือ

2.  ความต้องการการพยาบาลใน 24 ชั่วโมงของผู้ป่วยประเภทที่ 1,2,3,4 และ 5  เท่ากับ 1.36, 3.51, 5.12, 5.97 และ 6.15 ชั่วโมง ตามลำดับ

3.  การกระจายงานใน เวรเช้า : เวรบ่าย : เวรดึก  =   42 : 33 : 25

4.  จำนวนบุคลากรทางการพยาบาลที่ต้องการเพิ่ม จากผลการวิจัยครั้งนี้ รวม 25 คน

Abstract

The purposes of this research were to develop the Patient Classification System and determine the appropriate staffing in in-patient unit based on nursing care needs for the Naval Medical Department. Research samples consisted of the patients who admitted in wards of medical, surgical, pediatric, and obstetric and gynecological department in Somdej-prapinklao hospital and Somdej-pranangchaosirikitt hospital.  And 30 shifts of total nursing staffs in each department. Data Collected by using the 5 categories of Patient Classification System and nursing activities checklists for recording time using in each activities of  nursing care.  All checklists were tested for content validity and reliability scores were .80-1.00. Data collecting performed between September to November 2009.

The research results:

1. Having Patient Classification System for in-patient in medical, surgical,

pediatric, and obstetric and gynecological department of the Naval Medical Department.

2. The average of nursing time required in 24 hours for patients in category

1,2,3, 4,and 5 were 1.36 hours, 3.51 hours, 5.12 hours, 5.97 hours, and 6.15 hours respectively.

3. The workload distribution proportion in morning, evening, and night shift were 42 : 33 : 25

4. The numbers of nursing staffs required based on nursing care needs were 25.
