ประสบการณ์การปฏิบัติงานของพยาบาลหน่วยฉุกเฉินโรงพยาบาลระดับตติยภูมิ Working Experience of Emergency Nurses in Tertiary Care Hospital

Abstract

การศึกษาครั้งนี้เป็นการวิจัยเชิงคุณภาพตามแนวคิดปรากฏการณ์แบบตีความ มีวัตถุประสงค์เพื่อบรรยายประสบการณ์ การปฏิบัติงานของพยาบาลฉุกเฉิน คัดเลือกผู้ให้ข้อมูลแบบเฉพาะเจาะจงได้แก่ พยาบาลวิชาชีพในโรงพยาบาลตติยภูมิของรัฐ แห่งหนึ่งในเขตกรุงเทพมหานคร ที่มีประสบการณ์การปฏิบัติงานในหน่วยฉุกเฉินไม่น้อยกว่า 5 ปี จำนวน 12 คน รวบรวมข้อมูลด้วยการสัมภาษณ์แบบเจาะลึกและบันทึกเทป นำข้อมูลมาถอดแบบคำต่อคำ วิเคราะห์ข้อมูลตามวิธีการของ Crist and Tanner ผลการวิจัยมีดังนี้ 1. หน่วยงาน ER เป็นด่านหน้า ดูแลรักษาเวลาฉุกเฉิน 2. เลือกทำงานที่หน่วยฉุกเฉินด้วยลักษณะงานที่ท้าทายความสามารถ 3.จัดการปัญหาที่เกิดจากผู้รับบริการ ประกอบด้วย 4 ประเด็นย่อย คือ 3.1) ผู้ป่วยมากมาย อยากได้การดูแล แต่ต้องผ่านคัดกรองก่อนรับบริการทุกราย 3.2) วัยรุ่นตีกัน บาดเจ็บมา ER ต้องแยกรักษาอย่ามาเจอกัน 3.3) ผู้ป่วยจิตเวช ดูแลจิตใจเพิ่มพร้อมเรียกกำลังเสริม ก่อนเริ่มพฤติกรรมเสี่ยง และ 3.4) ผู้ป่วยไม่ฉุกเฉิน แต่อยากรับการรักษาไว มาใช้บริการ 4.ทำงานประสานหลายด้านมีเรื่องหลายประการที่ต้องแก้ไข ประกอบด้วย 4 ประเด็นย่อย ได้แก่ 4.1) OPD ส่งผู้ป่วยมาฝากไว้ บอกกล่าวเร็วไวจะได้เตรียมการ 4.2) การ Refer ผู้ป่วย ช่วยบอกล่วงหน้า จะได้จัดหาทรัพยากรได้ทัน 4.3) ขัดแย้งกับหอผู้ป่วยใน ขัดใจเรื่องส่งผู้ป่วยรักษาต่อ และ 4.4) แพทย์ต่างสั่งการรักษา ต้องขอให้มาปรึกษากัน

The purpose of this qualitative study is to describe work experience of emergency room nurses. The 12 emergency room nurses who have been working in a Tertiary hospital at least 5 years were selected by using purposive sampling technique. The in-depth interview with tape-record was used for data collection. All interviews were transcribed verbatim and analyzed by using content analysis of Crist and Tanner’s method. The research fidings are as follows:
1. The emergency room (ER) is the frontline of emergency care; 2. Working at ER is challenging; 3. Management in ER consists of 4 sub-themes including 3.1) Patients who visit ER will receive services after they were triaged, 3.2) In case of physical assaults the opponent must be separated while receiving treatment,  3.3) For patients with mental illnesses, the treatment has to be urgent to prevent adverse events, and 3.4) There are some non urgent patients who still require the urgent service; 4. Various problems that need to be solved including 4 sub-themes: 4.1) In patient department (IPD) should be well informed before transferring for effective preparation, 4.2) The referring should be informed in advance for preparing staff and others, 4.3) The conflct between emergency room and IPD about the patient’s transferring should be well managed, and 4.4) ER nurses might become confuse about difference treatment regimens from physicians from difference department, the good collaboration is strongly recommended.
