Development of Clinical Nursing Practice Guidelines for Pregnant Women with Preterm Labor Receiving Tocolytic Therapy in the Labor Unit, Nakhon Phanom Hospital

This research and development study aimed to develop and evaluate the effectiveness of Clinical Nursing Practice Guidelines (CNPG) for pregnant women with preterm labor receiving tocolytic therapy at the Labor Unit, Nakhon Phanom Hospital.The study was conducted in two phases: 1) development of the guidelines through the synthesis of seven research studies based on JBI criteria, and 2) pilot implementation with a purposively selected sample of 12 professional nurses and 30 pregnant women. The research instruments consisted of the CNPG, integrated with nursing process, and a five-part evaluation tool comprising: 1) general information, 2) nursing activity adherence, 3) quality of the guidelines, 4) user satisfaction, and 5) feasibility of implementation. The instruments were validated by three experts (CVI = 0.93) with a reliability coefficient (KR- 20) of 0.75. Data were analyzed using descriptive statistics. The findings revealed that professional nurses achieved a high adherence rate of over 93.33% during the initial and critical phases. Regarding clinical outcomes, none of the pregnant women experienced tocolytic related complications, and 83.33% successfully delayed labor for at least 48 hours. Furthermore, 100% of the nurses rated the guidelines as high quality and beneficial. The developed nursing practice guidelines are appropriate and should be implemented in labor units to enhance the quality of care for pregnant women with preterm labor receiving tocolytic therapy.
