Development of a Nursing Model to Prevent Hospital-acquired Pneumonia in Bedridden Elderly Patients at Pathum Thani Hospital

This research and development aimed to study the situation, develop a nursing model, and evaluate the results of using the nursing model to prevent Hospital-acquired Pneumonia among bedridden elderly patients at Pathum Thani Hospital. The purposive sample included three key informants, 43 professional nurses, and 142 bedridden elderly patients. The research was conducted in three phases: (1) Situation analysis, (2) Nursing model development, and (3) Evaluate the outcomes of nursing care models. The research instrument included (1) focus group, (2) nursing care model, (3) nursing practice questionnaires, (4) knowledge tests, (5) Self-efficacy perception questionnaires, and (6) satisfaction questionnaires. The content validity the instrument was verified by five experts, Content validity was verified with an Item Objective Congruence Index: IOC ranging from .8 to 1, with reliability coefficients of .95,.89,.93 and.95, respectively. Data were analyzed using descriptive statistics and a paired t-test.
The results revealed that the nursing model consists of three components: Assessment and surveillance of hospital-acquired pneumonia, nursing practice and nursing outcome. After using the hospital-acquired pneumonia assessment and monitoring scoring system, it reached the highest level. Nursing practice and Satisfaction with the nursing model were high level. The evaluation of the nursing model demonstrated a significant increase in mean knowledge and self-efficacy perception scores (p<.05) after implementation. No incidence of hospital-acquired pneumonia was found. In conclusion, the developed nursing care model effectively improved nurses’ knowledge, increase skills and confidence in preventing hospital-acquired pneumonia among bedridden elderly patients.
