Role of Nurse Case Manager at Short Stay Service and Chemotherapy in The Ramathibodi Home–based Chemotherapy Model : RHCM

The Ramathibodi Home Chemotherapy Model (RHCM) is an integrated process of cancer care for patients with colorectal cancer by a new multidisciplinary team approach. The patient is allowed to continue Chemotherapy administration at home. The case manager nurse plays a key and challenging role, possessing advanced nursing competencies in working with multidisciplinary teams. The goal is for patients to receive chemotherapy on schedule, improving treatment outcomes, ensuring safety, and enhancing patient satisfaction and quality of life. The nursing process is continuous, starting from before treatment, throughout the chemotherapy administration at home, and after care. Additionally, the case manager nurse has an essential role in nursing professional development and enhancing specialized nursing competencies by providing education, skills training, and mentoring for case manager nurses in network hospitals nationwide that implement this project.
