การประเมินผลการใช้หุ่นตรวจครรภ์ผิดปกติของ นักเรียนพยาบาลชั้นปีที่ 3 รุ่นที่ 48 วิทยาลัยพยาบาลกองทัพบก The Evaluation of Abnormal Pregnancy Model in Obstetrical Examination Among 3rd Year Nursing Student of Royal Thai Army Nursing College

Abstract

วิทยาลัยพยาบาลกองทัพบก ได้น้อมนำแนวคิดเศรษฐกิจพอเพียง ของพระบาทสมเด็จพระเจ้าอยู่หัวมาประดิษฐ์คิดค้น สื่อการสอนโดยนำวัตถุดิบที่มีอยู่ในประเทศ คือ ยางพารา สร้างเป็นนวัตกรรมหุ่นตรวจครรภ์ผิดปกติ โดยมีวัตถุประสงค์เพื่อประเมินความรู้ ทักษะ ความมั่นใจของนักเรียนภายหลังฝึกกับหุ่นตรวจครรภ์ผิดปกติ ที่ทำจากยางพารา และเครื่องกำเนิดเสียงเป็นวัสดุที่ผลิตในประเทศ มีทั้งหมด 3 แบบ คือ ครรภ์แฝด ครรภ์ท่าขวาง และครรภ์ที่มีภาวะน้ำเกิน โดยมีต้นทุนในการผลิตต่ำกว่าสั่งซื้อจากต่างประเทศ 5 เท่า และอยู่ในขั้นตอนของการขออนุมัติอนุสิทธิบัตร กระทรวงพาณิชย์หุ่นตรวจครรภ์ผิดปกติประกอบด้วย ตัวทารก ถุงน้ำคร่ำ และโครงสร้างต่าง ๆ ที่คล้ายคลึงกับหญิงตั้งครรภ์มากที่สุด หุ่นตรวจครรภ์ผิดปกติ สามารถฟังเสียงหัวใจทารก ประเมินอายุครรภ์ ท่าและส่วนนำของทารกในครรภ์ได้ นอกจากนี้ ผู้สอนสามารถตั้งค่า อายุครรภ์ ความเร็ว ช้า ของเสียงหัวใจทารก และส่วนนำต่างๆ ของหุ่นตรวจครรภ์ได้อีกด้วย โดยมี นักเรียนพยาบาลชั้นปีที่ 3 รุ่นที่ 47 จำนวน 76 นาย ทดลองใช้และประเมินผล ภายหลังจากฝึกกับหุ่นตรวจครรภ์ผิดปกติแล้ว พบว่าหุ่นตรวจครรภ์นี้มีประโยชน์กับนักเรียน ค่าเฉลี่ย 4.22 (S.D. = 0.53) มีประโยชน์กับท่านก่อนออกปฏิบัติงานต่างจังหวัด ค่าเฉลี่ย 4.21 (S.D. = 0.52) ทำให้นักเรียนมีความเข้าใจลักษณะของท้องและทารกในครรภ์ของหญิงคลอดที่มีความผิดปกติมากขึ้น และเข้าใจเทคนิควิธีการตรวจครรภ์จากการฝึกกับหุ่นตรวจครรภ์ค่าเฉลี่ยเท่ากันที่ 4.10 (S.D. = 0.55 และ 0.59 ตามลำดับ) และทำให้เกิดความมั่นใจในขั้นตอนของการตรวจครรภ์ ค่าเฉลี่ย 4.01 (S.D. = 0.71) จากคะแนน 5 ซึ่งโดยเฉลี่ย นักเรียนพยาบาลฝึกฝนด้วยตนเองกับหุ่นตรวจครรภ์ผิดปกติ จำนวน 3 ครั้งโดยสรุปหุ่นตรวจครรภ์ผิดปกติ มีประโยชน์และคุ้มค่ากับนักเรียนพยาบาล นอกจากนี้หุ่นตรวจครรภ์ผิดปกติจะถูกนำไปใช้เพื่อเพิ่มทักษะการตรวจครรภ์ให้กับสถาบันการศึกษาพยาบาลอื่น ๆ และพยาบาลวิชาชีพที่ต้องการฝึกทักษะเพื่อเพิ่มประสิทธิภาพของการฝากครรภ์ในหญิงตั้งครรภ์ในภูมิลำเนาที่ห่างไกล

Under the Suffiiency Economy Theory of His Majesty the King of Thailand, the department of obstetrics nursing, Royal Thai Army Nursing College has developed the Model of Abnormal Pregnancies aiming to evaluate knowledge skill and self-confience after practicing abnormal pregnancy models. The Models of Abnormal Pregnancies used local materials, such as rubber from local rubber trees, glues, and sound transformers to imitate high risk pregnant women, twins, poly-hydramios, and transverse line conditions. The costs of the all models are 5 times lower when compared to the export materials. The models contain fetus, amniotic sac, and other structure which similar to pregnant women. All of model can be monitored to fetal heart sound, evaluate of gestational age, position and presentation of fetus in the uterus. Moreover, the model can be set the gestational age, position, and presentation as instructors need to the students’ practice. After Practicing with models, nursing students have earned the Obstetrical examination skill. The evaluation by 76 3rd year students found that the models were useful by means 4.22 (S.D. = 0.53) and useful before fild practices means 4.21 (S.D = 0.52). Students were more understand about pregnancy anatomy and the skills of examination means 4.10 (S.D. = 0.55 and 0.59 consequently) and having self-confience in Antenatal care examination means 4.01 (S.D.= 0.71) from the total score 5. Furthermore, most students self-practiced with the model average 3 times. In conclusion, pregnancy models were useful for nursing students. Furthermore, the Models of Abnormal Pregnancies will be shared to other nursing institutes in order to increase the Obstetrical examination skills both nursing students and professional nurses. Also, the overview impacts will be raised the accessibility of effective Antenatal Care to unreached pregnant population to all the remote areas.
