การประเมินหลักสูตรพยาบาลกู้ชีพ โรงพยาบาลพระมงกุฎเกล้า An Evaluation of Emergency Medical Service Nursing Course at Phramongkutklao Hospital

Abstract

โรงพยาบาลพระมงกุฎเกล้าตระหนักถึงความสำคัญในการช่วยเหลือผู้ป่วยฉุกเฉิน รวมทั้งมีการวางแผนจัดตั้งศูนย์บริการการแพทย์ฉุกเฉิน เพื่อให้ข้าราชการทหาร ครอบครัว ประชาชนทั่วไป ได้รับการดูแลรักษาอย่างรวดเร็ว มีความปลอดภัย ลดอัตราการตายและความพิการ ซึ่งมีความจำเป็นอย่างยิ่งในการเตรียมบุคลากรในการปฏิบัติงานให้มีคุณภาพและประสิทธิภาพเกี่ยวกับการแพทย์ฉุกเฉิน จึงได้จัดการอบรมหลักสูตรพยาบาลกู้ชีพ ระยะเวลา 40 ชั่วโมง ซึ่งได้รับการรับรองหลักสูตร จาก สถาบันการแพทย์ฉุกเฉินแห่งชาติ  ดังนั้นเพื่อให้หลักสูตรมีการพัฒนาอย่างต่อเนื่องจึงควรมีการประเมินหลักสูตรและปรับปรุงให้สอดคล้องกับความต้องการและสังคมที่เปลี่ยนแปลงไป  วัตถุประสงค์ในการศึกษาครั้งนี้เพื่อประเมินหลักสูตรในด้านปัจจัยนำเข้า กระบวนการอบรมและผลผลิตของหลักสูตร โดยมีผู้เข้ารับการอบรมจำนวน 60 คน ตอบแบบสอบถามประเมินหลักสูตรแบบประมาณค่า 5 ระดับที่มีค่าความเชื่อมั่น .92 จากนั้นนำข้อมูลมวิเคราะห์โดยใช้ค่าเฉลี่ยและส่วนเบี่ยงเบนมาตรฐาน ผลการวิจัยพบว่า ผู้เข้ารับการอบรมเป็นพยาบาลเพศหญิง 43 คน เพศชาย 17 คน ปัจจัยนำเข้าที่มีค่าเฉลี่ยมากที่สุด ด้านผู้เข้ารับการอบรมพบว่าสมัครใจเข้ารับการ
อบรม  ( =  4.63)  ด้านวิทยากรพบว่า  มีคุณวุฒิและความสามารถ  ( =  4.67)  ด้านสิ่งสนับสนุนการเรียนรู้พบว่า  อุปกรณ์และเครื่องช่วยฝึกที่มีเหมาะสม ( = 4.42) สำหรับกระบวนการจัดอบรม พบว่า รูปแบบการจัดการอบรมเหมาะสมมีค่าเฉลี่ยมากที่สุด( = 4.37) ความเหมาะสมของเนื้อหาภาคทฤษฎีและภาคปฏิบัติเรื่องการประเมินสภาพผู้บาดเจ็บ มีค่าเฉลี่ยมากที่สุด (= 4.60 และ   =  4.55  ตามลำดับ)  สำหรับผลผลิตจากการอบรม  เมื่อเปรียบเทียบความรู้ก่อนและหลังเข้ารับการอบรม  พบว่า  ได้รับความรู้เรื่องการดูแลและจัดการทางเดินหายใจมากที่สุด  ( =  4.42)  เรื่องการปฏิบัติการดูแลหญิงตั้งครรภ์และเด็กน้อยที่สุด(= 3.98)

การประเมินหลักสูตรโดยรวมอยู่ในระดับดี  เป็นหลักสูตรที่มีความสำคัญมาก  เพราะพยาบาลกู้ชีพจะเป็นกลไกหนึ่งที่มีความสำคัญต่อการพัฒนาระบบการบริการการแพทย์ฉุกเฉิน  ทำให้การดูแลผู้ป่วยฉุกเฉินเป็นไปอย่างมีประสิทธิภาพมากยิ่งขึ้นแต่อย่างไรก็ตามผู้เข้ารับการอบรมได้เสนอแนะเพิ่มระยะเวลาในการอบรมและพิจารณาทบทวนเกี่ยวกับการปฏิบัติการดูแลหญิงตั้งครรภ์และเด็ก ทั้งนี้ผู้รับผิดชอบหลักสูตรควรนำมาเป็นแนวทางในการพัฒนาปรับปรุงเพื่อให้หลักสูตรสอดคล้องกับความต้องการ
ของสังคมและทำให้ผู้เข้ารับการอบรมปฏิบัติงานได้อย่างมีประสิทธิภาพ

Phramongkutklao Hospital has realized the importance of providing care to emergency patients and established Emergency Medical Service Center (EMS center) for safe and efficient care to military personnel, their family and general people, resulting in decreased mortality and mobility among the patients. Nursing staff preparedness is critical to improve quality and effectiveness of EMS system. Thus, the hospital arranged 40-hour
EMS nursing course affirmed by Emergency Medical Institute of Thailand to respond to the needs of skilled nurses to serve the these patients. The course needs to be continuously evaluated for ongoing improvement,
conforming to the needs of the changing society. This study aimed to evaluate input, process and output of the EMS course. 60 participant nurses rated their opinion on the course evaluation using five point Lickert scale in the questionnaires (reliability = .92). The collected data were analyzed using mean and standard deviations. The results found that 60 nurses participated in the EMS course, 43 were female and 17 were male. For the input, course participation volunteering had the highest mean in the participant aspect ( = 4.63). In the lecturer aspect, the highest mean was the qualification and ability (= 4.67). For learning support resources,
equipments and training-assisted materials were found appropriate ( = 4.42). In terms of process, course design had the highest mean ( = 4.37). The theoretical and practical content in initial assessment was rated
the most appropriate ( = 4.60 and 4.55 respectively). For the output, the most knowledge gained topic was airway management ( = 4.42) and the lowest was in the topic of practice in pregnant women and baby care
( = 3.98)

The overall course evaluation was in satisfied level. This course is very important for EMS nurses, the crucial component of EMS system, to increase effectiveness of care for emergency patients. However, the
course participants suggested increasing course length and improving knowledge and skills related to care of in pregnant women and babies. The course managers should take the evaluation results into account for
proper improvement to enhance optimum working effectiveness of the participants and meet their needs.
