บทคัดย่อ

     อายุที่เพิ่มขึ้นและการใช้อุปกรณ์ช่วยเดิน เป็นปัจจัยภายในและภายนอกที่มีความสัมพันธ์กับการหกล้มในผู้สูงอายุ การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาถึงความสัมพันธ์ระหว่างอายุและการใช้อุปกรณ์ช่วยเดินกับความเสี่ยงหกล้มของผู้สูงอายุของกลุ่มตัวอย่างที่มีอายุ 60 ปีขึ้นไป ในเขตอำเภอเมือง จังหวัดพิษณุโลก จำนวน 200 คน สามารถเดินได้อย่างน้อย 10 เมตรโดยไม่ใช้อุปกรณ์ช่วยเดิน และผ่านการคัดกรองสภาพสมองเบื้องต้น โดยใช้แบบทดสอบฉบับภาษาไทย (Mental State Examination T10) ประเมินความเสี่ยงหกล้มโดยการทดสอบเดินไปกลับระยะทาง 3 เมตร (Mental State Examination T10 (MSET10)) บันทึกข้อมูลของอายุ และการใช้อุปกรณ์ช่วยเดินจากการสัมภาษณ์ วิเคราะห์ผลการศึกษาแบบตัดขวางโดยใช้สถิติการถดถอยโลจิสติก (Logistic regression) เพื่อระบุค่าความสัมพันธ์ระหว่างอายุและการใช้อุปกรณ์ช่วยเดินกับความเสี่ยงหกล้ม (TUG) กำหนดค่านัยสำคัญทางสถิติที่ p-value < 0.05
      ผลการศึกษาพบว่า อายุและการใช้อุปกรณ์ช่วยเดินมีความสัมพันธ์กับความเสี่ยงต่อการหกล้มของผู้สูงอายุ โดยมีค่า Adjusted odds ratio เท่ากับ 1.06 (95% CI: 1.01, 1.12) และ 0.07 (95% CI: 0.02, 0.23) ตามลำดับ ผลการศึกษานี้สนับสนุนว่าเมื่ออายุเพิ่มขึ้น ย่อมเกิดความเสี่ยงต่อการหกล้มเพิ่มขึ้นตามมา นอกจากนี้สำหรับผู้สูงอายุที่มีความจำเป็นต้องใช้อุปกรณ์ช่วยเดิน การใช้อุปกรณ์ช่วยเดินสามารถลดความเสี่ยงต่อการหกล้มได้ จึงควรมีการประเมินความจำเป็นในการใช้อุปกรณ์ช่วยเดินในผู้สูงอายุ เพื่อเป็นแนวทางในการวางแผนส่งเสริมสุขภาพและป้องกันความเสี่ยงหกล้มในผู้สูงอายุต่อไป
