ลักษณะและปัจจัยเสี่ยงของพฤติกรรมการทำร้ายตนเองแบบไม่เจตนาให้เสียชีวิตในผู้ป่วยโรคซึมเศร้าเด็กและวัยรุ่น โรงพยาบาลลำพูน

บทคัดย่อ
วัตถุประสงค์ เพื่อศึกษาลักษณะและปัจจัยเสี่ยงของพฤติกรรมการทำร้ายตนเองแบบไม่เจตนาให้เสียชีวิตในผู้ป่วยโรคซึมเศร้าเด็กและวัยรุ่น โรงพยาบาลลำพูน และศึกษาความแตกต่างของลักษณะและปัจจัยเสี่ยงของพฤติกรรมการทำร้ายตนเองแบบไม่เจตนาให้เสียชีวิต ระหว่างกลุ่มผู้ป่วยอายุน้อยกว่า15 ปี และมากกว่าหรือเท่ากับ 15 ปี
วิธีการศึกษา การศึกษาภาคตัดขวาง (cross-sectional study) ในผู้ป่วยเด็กและวัยรุ่นที่ได้รับการวินิจฉัยโรคซึมเศร้าตาม DSM-5 อายุ 11 - 18 ปี และมีประวัติการทำร้ายตนเองอย่างน้อย 1 ครั้ง ระหว่างเดือนมกราคม - ธันวาคม พ.ศ. 2565 จำนวน 80 คน โดยใช้แบบสอบถามพฤติกรรมทำร้ายตนเอง The Brief Non-Suicidal Self-Injury Assessment Tool (BNSSI-AT) Thai version และแบบสอบถามปัจจัยเสี่ยงต่อการทำร้ายตนเอง ใช้สถิติเชิงพรรณนา, Fisher exact test และ t-test
ผลการศึกษา ผู้ป่วยโรคซึมเศร้าเด็กและวัยรุ่น โรงพยาบาลลำพูน ที่ทำร้ายตนเอง 80 คน อายุเฉลี่ย 14.9 ปี เป็นเพศหญิง ร้อยละ 88.7 อายุเฉลี่ยที่ทำร้ายตนเองครั้งแรก 13.1 ปี ความถี่ของพฤติกรรมทำร้ายตนเองแบบไม่เจตนาให้เสียชีวิตเฉลี่ย 25 ครั้งต่อปี ร้อยละ 56.3 ใช้วิธีการตั้งแต่ 5 วิธีขึ้นไป ร้อยละ 87.3 ใช้วิธีกรีดข้อมือและแขน ร้อยละ 58.7 มีความเป็นไปได้ที่จะทำร้ายตนเองซ้ำ เหตุผลของการทำร้ายตนเองแบบไม่เจตนาให้เสียชีวิตที่พบบ่อยที่สุดคือเพื่อระบายความเครียดและความกดดัน ผู้ป่วยส่วนใหญ่ตระหนักว่าต้องต่อสู้กับความรู้สึกที่อยากทำร้ายตนเอง และมองการทำร้ายตนเองเป็นปัญหาต่อชีวิต ร้อยละ 53.8 มองว่าการทำร้ายตนเองส่งผลกระทบต่อคุณค่าความภาคภูมิใจของตนเอง ผู้ป่วยอายุมากกว่าหรือเท่ากับ15 ปี มองว่าการทำร้ายตนเองส่งผลกระทบต่อการเลือกใส่เสื้อผ้ามากกว่าอย่างมีนัยสำคัญ (p = 0.039) ส่วนผู้ป่วยอายุน้อยกว่า 15 ปี มองว่าการทำร้ายตนเองไม่รบกวนชีวิตมากกว่าอย่างมีนัยสำคัญ (p = 0.035) ปัจจัยเสี่ยงของพฤติกรรมทำร้ายตนเองแบบไม่เจตนาให้เสียชีวิตที่พบบ่อยที่สุด ได้แก่ การเคยถูกเพื่อนกลั่นแกล้ง และการถูกวิพากษ์วิจารณ์โดยคนในครอบครัว
สรุป ผู้ป่วยโรคซึมเศร้าเด็กและวัยรุ่น โรงพยาบาลลำพูน พบว่าส่วนใหญ่ทำร้ายตัวเองด้วยใช้วิธีกรีดแขน เพื่อระบายความเครียด ความกดดัน ปัจจัยเสี่ยงที่พบบ่อย ได้แก่ การถูกกลั่นแกล้งและถูกวิจารณ์โดยคนในครอบครัว ปัจจัยที่ทำให้ผู้ป่วยตระหนักรู้คือการต้องต่อสู้กับความรู้สึกอยากทำร้ายตนเอง และทราบว่าการทำร้ายตัวเองกระทบต่อคุณค่าและความภาคภูมิใจในตนเอง
