ผลการรักษาผู้ป่วยที่มีภาวะถอนพิษสุราแบบผู้ป่วยในของโรงพยาบาลในเครือข่ายโรงพยาบาลสุรินทร์

บทคัดย่อ
วัตถุประสงค์ เพื่อศึกษาผลการรักษาผู้ป่วยที่มีภาวะถอนพิษสุราแบบผู้ป่วยในของโรงพยาบาลในเครือข่ายโรงพยาบาลสุรินทร์ ได้แก่ ปริมาณยา Diazepam ที่ผู้ป่วยได้รับระหว่างการรักษาในแผนกผู้ป่วยใน และระยะเวลาหายจากภาวะถอนพิษสุรา
วิธีการศึกษา การศึกษาย้อนหลังจากข้อมูลเวชระเบียนผู้ป่วยในของโรงพยาบาลในเครือข่ายโรงพยาบาลสุรินทร์ระหว่างเดือนตุลาคม 2565 - กันยายน 2566 จำนวน 140 คน วิเคราะห์ข้อมูลทั่วไปของผู้ป่วยด้วยสถิติเชิงพรรณนา หาอัตราระยะเวลาหายจากอาการจาก Kaplan-Meier method และทดสอบด้วย Log-rank test
ผลการศึกษา ผู้ป่วยทั้งหมดเป็นเพศชาย อายุเฉลี่ย 40.58 ± 10.39 ปี เป็นผู้ดื่มแบบติด ร้อยละ 75 ภาวะแทรกซ้อนที่พบมากที่สุดคือ ภาวะเพ้อสับสน (ร้อยละ 55) เกลือแร่โพแทสเซียมต่ำ (ร้อยละ 45.71) แมกนีเซียมต่ำ (ร้อยละ 43.57) และตับอักเสบ (ร้อยละ 22.86) ผู้ป่วยมีระยะเวลาภาวะถอนพิษสุราเฉลี่ย 3.27 ± 1.39 วัน ในการติดตาม 5 วัน พบผู้ป่วยหายจากภาวะถอนพิษสุรา 98 ราย มีค่ามัธยฐานระยะเวลาหายจากอาการ 4 วัน (95% CI: 4.0 - 5.0) ระยะเวลาการหายแตกต่างกันตามความรุนแรงของอาการอย่างมีนัยสำคัญ กลุ่มอาการรุนแรงมากได้รับยา Diazepam สูงสุด (195.14 ± 80.95 mg)
สรุป ผู้ป่วยส่วนใหญ่เป็นผู้ดื่มแบบติดและมีภาวะแทรกซ้อนสำคัญ ค่ามัธยฐานระยะเวลาหายจากภาวะถอนพิษสุราคือ 4 วัน ระยะเวลาและปริมาณยาที่ใช้มีความสัมพันธ์โดยตรงกับความรุนแรงของอาการ ข้อมูลนี้มีประโยชน์ต่อการวางแผนการรักษา การเฝ้าระวังภาวะแทรกซ้อน และการจัดเตรียมทรัพยากรในการดูแลผู้ป่วยที่มีภาวะถอนพิษสุรา
