บทคัดย่อ
วัตถุประสงค์ เพื่อศึกษาความชุกและปัจจัยที่สัมพันธ์กับพฤติกรรมทำร้ายตนเองในผู้ป่วยโรคซึมเศร้าเด็กและวัยรุ่น โรงพยาบาลสมุทรปราการ
วิธีการศึกษา การศึกษานี้เป็นการศึกษาเชิงวิเคราะห์โดยเก็บข้อมูลย้อนหลัง ในผู้ป่วยจิตเวชเด็กและวัยรุ่นโรคซึมเศร้ารายใหม่ที่มารับการรักษาที่แผนกจิตเวชและยาเสพติด โรงพยาบาลสมุทรปราการทั้งหมด ตั้งแต่เดือนมกราคม พ.ศ. 2564 ถึงเดือนธันวาคม พ.ศ. 2566 จำนวน 257 ราย โดยเก็บรวบรวมข้อมูลทั่วไปของผู้ป่วยและครอบครัว ประวัติความเจ็บป่วย ลักษณะพฤติกรรมทำร้ายตนเอง และแบบคัดกรองภาวะซึมเศร้า Children’s Depression Inventory (CDI) ในผู้ป่วยอายุน้อยกว่า 15 ปี แบบคัดกรองโรคซึมเศร้า 9 คำถาม (9Q) ในผู้ป่วยอายุ 15 - 18 ปี วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา chi-square test, t-test, Fisher’s Exact test, Mann-Whitney U test, binary logistic regression
ผลการศึกษา ผู้ป่วยเด็กและวัยรุ่นโรคซึมเศร้าทั้งหมด 257 ราย ค่าอายุมัธยฐาน 15 ปี (P25-P75 = 13.75 - 17 ปี) พบความชุกพฤติกรรมทำร้ายตนเอง ร้อยละ 59.9 วิธีการทำร้ายตนเองที่พบมากที่สุด ได้แก่ การใช้ของมีคม ร้อยละ 69.5 กินยาเกินขนาด ร้อยละ 37.2 ทุบตีตนเอง ร้อยละ 25.3 เจตนาทำร้ายตนเองที่พบมากที่สุด ได้แก่ ระบายอารมณ์ ร้อยละ 85.1 ตั้งใจฆ่าตัวตาย/เสียชีวิต ร้อยละ 44.2 ประชด/เรียกร้องความสนใจ ร้อยละ 8.4 ปัจจัยที่สัมพันธ์กับพฤติกรรมทำร้ายตนเองในผู้ป่วยโรคซึมเศร้าเด็กและวัยรุ่นอย่างมีนัยสำคัญทางสถิติ ได้แก่ มีความคิดอยากตายหรือความคิดฆ่าตัวตาย (OR = 3.091, 95% CI = 1.749 - 5.462, p <0.001) และระดับความรุนแรงของอาการซึมเศร้า (OR = 3.569, 95% CI 1.908 - 6.675, p<0.001)
สรุป ความชุกของพฤติกรรมทำร้ายตนเองในผู้ป่วยโรคซึมเศร้าเด็กและวัยรุ่นโรคซึมเศร้าโรงพยาบาลสมุทรปราการพบได้ ร้อยละ 59.9 ปัจจัยการมีความคิดอยากตายหรือความคิดฆ่าตัวตาย และระดับความรุนแรงของโรคซึมเศร้าสัมพันธ์กับความเสี่ยงของพฤติกรรมทำร้ายตนเองในผู้ป่วยกลุ่มดังกล่าว
