บทคัดย่อ
วัตถุประสงค์ เพื่อประเมินคุณภาพชีวิตของเด็กและวัยรุ่นที่มีพัฒนาการระบบอวัยวะเพศผิดปกติ และตรวจสอบความสอดคล้องระหว่างคุณภาพชีวิตที่ประเมินโดยเด็กและผู้ปกครอง
วิธีการศึกษา เป็นการศึกษาเชิงพรรณนา ณ ช่วงเวลาใดเวลาหนึ่ง โดยเก็บข้อมูลจากเด็กที่มีพัฒนาการระบบอวัยวะเพศผิดปกติที่มารับการรักษาแบบผู้ป่วยนอกที่คลินิกต่อมไร้ท่อ ฝ่ายกุมารเวชศาสตร์ โรงพยาบาลจุฬาลงกรณ์ ระหว่างเดือนตุลาคม 2565 ถึงเดือนพฤษภาคม 2566 โดยใช้แบบสอบถาม Pediatric Quality of Life Inventory™ 4.0 Generic Core Scale (PedsQL™) ในการประเมินคุณภาพชีวิตของอาสาสมัคร อายุ 8 - 18 ปี จำนวน 47 ราย ผู้ตอบแบบสอบถามคือผู้ปกครองและเด็กที่มีพัฒนาการระบบอวัยวะเพศผิดปกติ วิเคราะห์และนำเสนอข้อมูลทางสถิติด้วยค่าความถี่ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติ Independent และ Dependent T-test กำหนดค่านัยสำคัญทางสถิติที่ P < 0.05
ผลการศึกษา อาสาสมัครเด็กมีอายุเฉลี่ยและค่าเบี่ยงเบนมาตรฐานอยู่ที่ 13.18 ± 3.24 ปี โดยร้อยละ 51.1 ของอาสาสมัครระบุตนเองเป็นเพศชาย มีค่าเฉลี่ยและส่วนเบี่ยงเบนมาตรฐานของคะแนนคุณภาพชีวิตรวมเท่ากับ 87.58 ± 9.92 คะแนน เมื่อประเมินโดยเด็กและวัยรุ่นที่มีพัฒนาการระบบอวัยวะเพศผิดปกติ และ 72.06 ± 17.94 คะแนน เมื่อประเมินโดยผู้ปกครองของเด็ก โดยผลการศึกษาพบว่า คะแนนคุณภาพชีวิตรวมที่ประเมินโดยเด็กมีค่าเฉลี่ยมากกว่าที่ประเมินโดยผู้ปกครองอย่างมีนัยสำคัญทางสถิติ (mean difference = 15.52, t(47) = 6.292, P <.001) โดยผู้ปกครองประเมินคุณภาพชีวิตของเด็กและวัยรุ่นที่มีพัฒนาการระบบอวัยวะเพศผิดปกติแย่กว่าที่เด็กประเมินในทุกมิติคุณภาพชีวิต
สรุป เด็กและวัยรุ่นที่มีพัฒนาการระบบอวัยวะเพศผิดปกติมีมุมมองต่อคุณภาพชีวิตของตนเองดีกว่าที่ผู้ปกครองประเมิน จึงเป็นข้อพิจารณาสำหรับแพทย์ผู้รักษาในการเข้าใจคุณภาพชีวิตของเด็กและวัยรุ่นกลุ่มนี้ เพื่อนำไปสู่การวางแผนการรักษาดูแลได้อย่างเหมาะสม
