ความชุกและปัจจัยที่สัมพันธ์กับความเสี่ยงการฆ่าตัวตายในผู้ป่วยโรคซึมเศร้า โรงพยาบาลเจริญกรุงประชารักษ์

บทคัดย่อ
วัตถุประสงค์ เพื่อศึกษาความชุกของผู้ที่มีความเสี่ยงการฆ่าตัวตายในผู้ป่วยโรคซึมเศร้า รวมถึงปัจจัยที่สัมพันธ์กับความเสี่ยงการฆ่าตัวตายในผู้ป่วยโรคซึมเศร้า ในโรงพยาบาลเจริญกรุงประชารักษ์
วิธีการศึกษา การศึกษานี้เป็นเป็นการศึกษาเชิงพรรณนาและวิเคราะห์แบบย้อนหลัง ในกลุ่มผู้ป่วยช่วงอายุ 20 – 60 ปี ที่ได้รับการวินิจฉัยโรคซึมเศร้าเป็นครั้งแรก และอยู่ในช่วง 6 เดือนแรกของการรักษา ที่มารับบริการในโรงพยาบาลเจริญกรุงประชารักษ์ ในปี พ.ศ 2565 เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามข้อมูลทั่วไป แบบคัดกรองโรคซึมเศร้า 9 คำถาม (9Q) และแบบประเมินการฆ่าตัวตาย 8 คำถาม (8Q) วิเคราะห์ข้อมูลโดยใช้ chi-square test, Fisher’s Exact test, Kruskal-Wallis test, binary logistic regression และ multivariable logistic regression
ผลการศึกษา ผู้ป่วยซึมเศร้าเข้าร่วมการวิจัยจำนวน 441 คน มีค่ามัธยฐานของอายุเท่ากับ 31 ปี (IQR = 28) ผู้ป่วยส่วนใหญ่อยู่ในช่วงอายุ 20 – 30 ปี จำนวน 211 คน (ร้อยละ 47.9) เพศหญิงร้อยละ 73.9 พบว่าร้อยละ 72.8 อยู่ในสถานภาพไร้คู่ และว่างงานร้อยละ 58.7 พบความชุกของความเสี่ยงการฆ่าตัวตายร้อยละ 64.0 ในกลุ่มนี้แบ่งเป็น ผู้ที่มีความคิดอยากฆ่าตัวตายร้อยละ 37.9 และผู้ที่พยายามฆ่าตัวตายร้อยละ 26.1 ปัจจัยที่มีความสัมพันธ์กับความเสี่ยงการฆ่าตัวตายอย่างมีนัยสำคัญทางสถิติโดยการวิเคราะห์แบบ multivariable logistic regression พบ 2 ปัจจัยได้แก่ เป็นโรคซึมเศร้าระดับรุนแรง (AOR 104.09, 95% CI 43.51-249.04) และการทำให้ตัวเองบาดเจ็บ (AOR 65.15, 95% CI 21.44-197.97)
สรุป จากการศึกษาพบว่าปัจจัยที่มีความสัมพันธ์กับความเสี่ยงการฆ่าตัวตายในผู้ป่วยซึมเศร้าได้แก่ การทำให้ตัวเองบาดเจ็บ และเป็นโรคซึมเศร้าระดับรุนแรง จึงเป็นปัจจัยสำคัญที่ต้องตระหนักในการประเมินผู้ป่วยซึมเศร้า และอาจเป็นแนวทางที่มีประสิทธิภาพในการป้องกันการฆ่าตัวตายในผู้ป่วยซึมเศร้าได้
