การศึกษาความชุกและปัจจัยที่มีผลต่อการกินยาไม่สม่ำเสมอของผู้ป่วยที่เข้ารับการรักษาในคลินิก จิตเวชเด็กและวัยรุ่น โรงพยาบาลนครนายก

บทคัดย่อ
วัตถุประสงค์ เพื่อศึกษาความชุกและปัจจัยที่สัมพันธ์กับการกินยาไม่สม่ำเสมอในผู้ป่วยจิตเวชเด็กและวัยรุ่นโรงพยาบาลนครนายก
วิธีการศึกษา เป็นการศึกษาเชิงพรรณนาแบบสำรวจภาคตัดขวาง (Cross-sectional descriptive survey) เก็บข้อมูลผู้ป่วยอายุ 5 - 18 ปี ที่มารับการรักษาแบบผู้ป่วยนอกคลินิกจิตเวชเด็กและวัยรุ่น โรงพยาบาลนครนายก ในช่วงเดือนมกราคม - เมษายน พ.ศ. 2565 จำนวน 71 ราย เก็บข้อมูลทั่วไปของผู้ป่วยและผู้ปกครอง ข้อมูลเกี่ยวกับการรักษา และการกินยาของผู้ป่วย
ผลการศึกษา มีผู้ตอบแบบสอบถาม 71 ราย เป็นเพศชาย (ร้อยละ 63.4) อายุเฉลี่ย 11.9±4.1 ปี ส่วนใหญ่ศึกษาชั้นประถมศึกษา (ร้อยละ 50.7) ได้รับการวินิจฉัยเป็นโรคสมาธิสั้น (ร้อยละ 56.3) ยาที่ใช้ส่วนใหญ่เป็น Methylphenidate (ร้อยละ 56.3) ระยะเวลาเฉลี่ยที่เข้ารับการรักษาเท่ากับ 20.0±14.9 เดือน พบความชุกของผู้เข้าร่วมการวิจัยกินยาไม่สม่ำเสมอร้อยละ 49.3 สาเหตุหลักที่กินยาไม่สม่ำเสมอคือลืมกินยา (ร้อยละ 28.1) รองลงมาคือยาหมดเนื่องจากไม่มาตามนัด (ร้อยละ 21.1) และผู้ป่วยไม่ยอมกินยา (ร้อยละ 15.5) ไม่พบปัจจัยที่มีความสัมพันธ์กับการกินยาไม่สม่ำเสมอ แต่พบปัจจัยที่มีผลต่อการกินยาสม่ำเสมอคือช่วงอายุ 5 - 10 ปี (p-value = 0.03) และมีภูมิลำเนาอยู่ในเขตจังหวัดนครนายก (p-value = 0.03)
สรุป ผลการศึกษาที่ได้ควรนำไปใช้ประโยชน์ในการเฝ้าระวังทางคลินิกและพัฒนาเครื่องมือหรือโครงการที่ช่วยให้ผู้ป่วยกินยาสม่ำเสมอมากขึ้น
