การรักษาอาการปวดในผู้ป่วยโรคไตเรื้อรัง

บทคัดย่อ
อาการปวดเป็นกลุ่มอาการที่พบได้บ่อยในผู้ป่วยโรคไตเรื้อรัง ซึ่งหากไม่ได้รับการดูแลที่เหมาะสมจะนำไปสู่ผลเสียอย่างอื่นที่กระทบต่อคุณภาพชีวิตที่ดีของผู้ป่วย เช่น การนอน การทำกิจกรรมต่างๆในชีวิตประจำวัน การออกไปทำงานหาเลี้ยงชีพ หรือในผู้ป่วยที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียม อาจจะทำให้ผู้ป่วยต้องขาดการฟอกเลือดเนื่องจากไม่สามารถเดินทางได้สะดวก ทั้งนี้หากจะดูแลเรื่องอาการปวดให้ได้เหมาะสมควรมีการประเมินอาการปวดที่เหมาะสมใน 4 ปัจจัย คือความรุนแรงของอาการปวด ระยะเวลาของอาการปวด ชนิดของอาการปวดและเป้าหมายในการรักษาอาการปวด เพื่อให้ทั้งแพทย์และคนไข้มีส่วนร่วมในการรักษาอาการปวดทำให้ได้ผลการรักษาที่ดียิ่งขึ้น ทั้งนี้ปัจจุบันมียาที่ใช้รักษาอาการปวดหลักๆ คือยาพาราเซตามอล ยาต้านการอักเสบชนิดที่ไม่ใช่สเตียรอยด์ (NSAIDs) และยาระงับปวดกลุ่มโอปิออยด์ (opioid) โดยยาทั้งสามกลุ่มนี้จะมีเภสัชจลนศาสตร์ที่เปลี่ยนไปและผลข้างเคียงที่พบได้มากขึ้นในผู้ป่วยที่มีภาวะการทำงานของไตบกพร่อง การเลือกใช้ยาแนะนำให้พิจารณาจาก 4 ปัจจัยที่กล่าวไปเบื้องต้น โดยเริ่มจากการใช้ยาพาราเซตามอลในผู้ที่มีอาการปวดน้อย หากอาการไม่ดีขึ้นหรือผู้ป่วยมีอาการปานกลางจนถึงปวดรุนแรงค่อยพิจารณาเรื่องการใช้ยาต้านการอักเสบชนิดที่ไม่ใช่สเตียรอยด์หรือยาระงับปวดกลุ่มโอปิออยด์ต่อไป
