บทคัดย่อ

บทคัดย์อ: ได้ทำการศึกษาย้อนหลังผู้ป่วย parororoxysmal noctumal hemoglobinuria (PNIH) จ้านวน 51 ราย ที่รับไว้รักษาในหน่วยโลหิตวิทยา ภาควิชาอายุรศาสตร์ มหาวิทยาลัยเชียงใหม่ ระหว่างปี พ.ศ. 2516 ถึง 2540 เปรียบเทียบกับรายงานจากร.พ.ศิริราช จีน ทวีปยุโรป และอเมริกา พบว่าผู้ป่วย PNH ของเชียงใหม่มีลักษณะหลายอย่างคล้ายคลึงกับรายงานจากแถบเอเซีย (ร.พ.ศิริราช และจีน) แต่แตกต่างกับรายงานจากทวีปยุโรป และอเมริกา คือ พบในเพศชายมากกว่าเพศหญิง พบในอายุที่น้อยกว่า เป็นโรค aplastic anemia/dyshemopoiesis บ้ามาก่อนสัตส่วนมากกว่า (ยกเว้นรายงานของฝรั่งเศส) ผู้ป่วยเสียชีวิตจาก thrombosis น้อยกว่า แต่เสียชีวิตจากการติดเชื้อมากกว่า ส่วนข้อแตกต่างของผู้ป่วย PNH เชียงไหมกับ รายงานจากศิริราช คือ ผู้ป่วยเชียงใหม่
มีอาการเลือดออก และตรวจพบ severe thrombocytopenia (platelet count < 50,000/mm²) น้อยกว่า
