Transfusion in Rare Blood Type, Ko with Anti-Ku : A Case Report

บทคัดย่อ : รายงานผู้ป่วย 1 ราย หญิงไทยคู่ อายุ 69 ปี มาโรงพยาบาลด้วยอาการเจ็บหน้าอก ผลการตรวจพบว่ามีเส้นเลือดหัวใจตึบ ในการเตรียมเลือดเพื่อการผ่าตัดตรวจพบว่ามีหมู่เลือด B, Rh posive และมี anti-Leb แต่เมื่อนำเลือด Le(a-b-) จำนวน 8 ยูนิตมาทำ crossmatch (IAT) พบว่าให้ผลบวกอย่างอ่อน เมื่อดูด้วยกล้องจุลทรรศน์ การทำ antibody screening ให้ผลบวกกับ screen cells เช่นเดียวกับการทำ crossmatch เมื่อทํา antibody identication
(LAT) ได้ผล 1+ กับทุก panel cells โดยที่ autocontol ให้ผลลบ จากการตรวจอย่างละเอียดพบว่าผู้ป่วยมีหมู่เลือดชนิดทาได้ยากในคนไทย คือ Ko และมี anบ-Kน การผ่าตัดหัวใจในผู้ป่วยรายนี้สำเร็จลงได้ด้วยความช่วยเหลือที่ได้รับจากศูนย์บริการโลหิตแห่งชาติ สภากาชาดไทย ซึ่งได้ประสานงานไปยังสถาบันที่มีเลือด Ko และ Japanese Red Cross Society เป็นผู้จัดส่งเลือดที่ต้องการซึ่งก็บแช่แข็งไว้มาให้ใช้ในการผ่าตัด
