Pulmonary Hypertension in Hemophiliac HIV Positive Patient Receiving Long Term Prothrombin Complex Concentrate Therapy: Report of a Case and Review Literature

บทคัดย่อ: รายงานผู้ป่วย ชายไทย อายุ 35 ปี เป็น hemophilia B และติดเชื้อ HIV ผู้ป่วยได้รับการรักษาด้วย prothrombin complex concentration เป็นระยะเวลา 2 ปี จึงเกิดอาการหอบเหนือยเรือรังจาก nght heart failure ในระยะสุดท้ายผู้ป่ายมี severe pulmonary hypertension และหัวใจวายเฉียบพลันเพราะการใช้ protuombin complex concentrate จำนวนมากในระยะเวลา 7 วัน ผู้ป่วยได้รับการรักษาด้วย low molecular weight heparin ในขนาดต่ำร่วมกับพลาสมาสดแช่แข็ง ปรากฏว่าสามารถทำให้ผู้ป่วยมีอาการทุเลาลงโดยควบคุมภาวะแทรกช้อนจากเลือดออกได้ดี ผู้รายงานมีความเห็นว่า bepariก ขนาดน้อยน่าจะมีบหบาทช่วยลดความรุนแรงของ thromboembolism ในผู้ป่วย hemophila และโดยการใช้ FFFP ร่วมด้วยเป็นระยะ สามารถควบคุมภาวะแทรกซ้อนจากเลือดออกได้ ได้วิจารณ์สาเหตุของ thromboembolism จาก HIV ua: prothrombin complex concentrate ร่วมกับแนวทางการรักษาป้องกันภาวะดังกล่าวตามที่มีผู้รายงานไว้
