T-cell Leukemia presents with Enterocutaneous Fistula and Hemophagocytic Syndrome

บทคัดย่อ: รายงานผู้ป่วยเด็กหญิงไทยมุสลิมอายุ 3 ปี มาโรงพยาบาลด้วยอาการผีที่หลังด้านขวาจาก enterocutaneous
fistula และ pancytopenia จาก hemophagocytic syndrome ร่วมกับตรวจพบการติดเชื้อ Ebstein-Barr virus
หลังจากผ่าตัดเอาหนองออกและได้รับยาปฏิชีวนะรักษาการติดเชื้อในช่องท้อง ผู้ป่วยมีอาการดีขึ้นและผลเลือดกลับมา
เป็นปกติอยู่ 2 เดือน จึงเริ่มมีการอักเสบที่แผลผ่าตัดเดิม ร่วมกับมี pancytopenia อีก และพบเซลล์ตัวอ่อนในกระแส
เลือด ผลการตรวจไขกระดูกพบ Iymphoblasts มากกว่าร้อยละ 90 ของเซลล์ไขกระดูก ซึ่งย้อมติด T-cell marker
ได้รับการวินิจฉัยเป็น T-cell leukemia รักษาด้วย COMP protoco! อยู่ 1 ปี โรคกลับเป็นใหม่ ได้ให้การรักษาด้วย
ยาเคมีบำบัดใหม่จนครบ induction phase ผู้ป่วยไม่มารับการรักษาต่อ 2 เดือนต่อมาผู้ป่วยกลับมาด้วยโรคติดเชื้อ
ส่วนโรคมะเร็งเม็ดเลือดขาวไม่มี remission
