บทคัดย่อ
บทนำ : การส่งเสริมการดูแลแบบแกงการูมีความสำคัญต่อทารกเกิดก่อนกำหนด จึงจำเป็นต้องพัฒนาแนวปฏิบัติจากหลักฐานเชิงประจักษ์ให้เหมาะสมกับบริบทของหน่วยงาน
วัตถุประสงค์ : เพื่อศึกษาผลของแนวปฏิบัติการดูแลแบบแกงการู ต่อทัศนคติและพฤติกรรมของพยาบาลในการส่งเสริมการดูแลแบบแกงการู
วิธีดำเนินการวิจัย : การวิจัยนี้เป็นการวิจัยกึ่งทดลองแบบหนึ่งกลุ่มวัดก่อนและหลัง กลุ่มตัวอย่างคือพยาบาลวิชาชีพหอผู้ป่วยวิกฤตทารกแรกเกิด จำนวน 27 คน ผู้วิจัยพัฒนาแนวปฏิบัติฯ ตามกรอบแนวคิด PARIHS จากหลักฐานเชิงประจักษ์ และจัดอบรมเชิงปฏิบัติการร่วมกับการใช้วิดีทัศน์ สื่อคอมพิวเตอร์ และสถานการณ์จำลอง เก็บข้อมูลด้วยแบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามทัศนคติ และแบบประเมินพฤติกรรม วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา paired t-test, Wilcoxon Signed-Rank Test, simple linear regression และ multiple regression analysis
ผลการวิจัย : หลังการใช้แนวปฏิบัติฯ ค่าเฉลี่ยคะแนนทัศนคติต่อการดูแลแบบแกงการูของพยาบาลเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (t=11.73, p<.001) และคะแนนพฤติกรรมในการส่งเสริมการดูแลแบบแกงการูของพยาบาลหลังการใช้แนวปฏิบัติฯ ครั้งที่ 2 มีค่ามัธยฐานสูงกว่าครั้งที่ 1 อย่างมีนัยสำคัญทางสถิติ (Z=4.06, p<.001) ทั้งนี้คะแนนทัศนคติสามารถทำนายคะแนนพฤติกรรมการส่งเสริมการดูแลแบบแกงการูของพยาบาลได้อย่างมีนัยสำคัญทางสถิติ (F=8.383, p=.002)
สรุปผล : แนวปฏิบัติการดูแลทารกแบบแกงการูมีประสิทธิผลต่อการเพิ่มทัศนคติและพฤติกรรมของพยาบาลในการดูแลทารกแบบแกงการู
