บทคัดย่อ
บทนำ : ผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลันชนิด เอสที ยกสูง ที่ได้รับการรักษาด้วยการเปิดขยายหลอดเลือดหัวใจ อาจเกิดภาวะแทรกซ้อนจากโรคและการรักษา
วัตถุประสงค์การวิจัย : เพื่อศึกษาสถานการณ์ พัฒนาแนวปฏิบัติและศึกษาผลลัพธ์การใช้แนวปฏิบัติการพยาบาลผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลันชนิด เอสที ยกสูง
วิธีดำเนินการวิจัย : การศึกษานี้เป็นการวิจัยและพัฒนา ดำเนินการวิจัย 3 ระยะ คือ 1) ศึกษาสถานการณ์ 2) พัฒนาแนวปฏิบัติ 3) ประเมินผลลัพธ์การใช้แนวปฏิบัติ ตัวอย่าง คือ พยาบาลในหอผู้ป่วยหนักอายุรกรรม จำนวน 15 คน และผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลันชนิด เอสที ยกสูง ระดับ I และ II ที่ได้รับการเปิดขยายหลอดเลือดหัวใจชนิดปฐมภูมิ จำนวน 30 คน วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณา สถิติทดสอบวิลคอกซัน การทดสอบค่าที และการทดสอบของฟิชเชอร์
ผลการวิจัย : 1) สถานการณ์การพยาบาลพบว่า (1) ผู้ป่วยเกิดเลือดออกหรือก้อนเลือดบริเวณที่ทำหัตถการ และจำนวนวันนอนโรงพยาบาล 6-8 วัน (2) แนวปฏิบัติการพยาบาลยังไม่ครบทุกมิติ การวางแผนจำหน่ายไม่เป็นแนวทางเดียวกัน 2) แนวปฏิบัติที่พัฒนาขึ้น มี 6 ระยะ และ 3) ผลลัพธ์หลังใช้แนวปฏิบัติ พบว่า ความรู้ของพยาบาลเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ภาวะเลือดออกหรือก้อนเลือดใต้ผิวหนังบริเวณที่ทำหัตถการ และจำนวนวันนอนโรงพยาบาลลดลงกว่าเดิมอย่างมีนัยสำคัญทางสถิติ (p<0.05) ความพึงพอใจของพยาบาลและผู้ป่วยอยู่ในระดับมากที่สุด (=4.79, 4.61; SD=0.15, 0.39 ตามลำดับ)
สรุปผล : แนวปฏิบัติการพยาบาลตั้งแต่แรกรับจนถึงจำหน่ายที่มีมาตรฐาน ทำให้การดูแลผู้ป่วยโรคกล้ามเนื้อหัวใจตายเฉียบพลันชนิด เอสที ยกสูง เกิดผลลัพธ์ที่ดี
