บทคัดย่อ
บทนำ: บัญชียาหลักแห่งชาติได้ถูกนำมาใช้ในปี 2524 โดยกระทรวงสาธารณสุข มีจุดประสงค์เพื่อควบคุมค่าใช้จ่ายในการดูแลสุขภาพ ยานอกบัญชียาหลักหมายถึงยาที่ไม่ได้มีรายชื่ออยู่ในบัญชีดังกล่าว ยานอกบัญชียาหลักควรจะมีการเข้มงวดในการสั่งใช้เพราะยามีราคาแพงและบางครั้งสามารถใช้ยาในบัญชีแทนได้ การประเมินการใช้ยานอกบัญชียาหลักครั้งนี้มุ่งเน้นที่ยาในกลุ่มตัวต้านตัวรับแองจิโอเทนซิน ทู (Non-ED ARBs) ที่โรงพยาบาลดำเนินสะดวก โดยมีจุดประสงค์เพื่อประเมินความสอดคล้องของการสั่งใช้ยาในกลุ่ม Non-ED ARBs ตามเกณฑ์ของการสั่งใช้ยาในด้านข้อบ่งใช้และข้อห้ามใช้ที่จัดทำขึ้น วิธีดำเนินการวิจัย: เกณฑ์การประเมินการสั่งใช้ยาในกลุ่ม Non-ED ARBs พัฒนาขึ้นจากเอกสารอ้างอิงทางวิชาการและแนวทางการสั่งใช้ยา นอกบัญชียาหลักของกระทรวงสาธารณสุข ทำการรวบรวมข้อมูลย้อนหลังจากเวชระเบียนฐานข้อมูลผู้ป่วยและแบบประเมินการสั่งใช้ยาของโรงพยาบาลดำเนินสะดวกโดยเก็บข้อมูลทั้งหมด 3 ปี (ตั้งแต่ 1 ตุลาคม 2554 - 30 กันยายน 2557) ผลการวิจัย: มีการสั่งใช้ยาในกลุ่ม Non-ED ARBs ทั้งหมด ในช่วงที่ทำการศึกษาคือ 260 ใบสั่งยา โดยพบว่า มี 7 (ร้อยละ 2.69) และ 36 (ร้อยละ13.84) ใบสั่งยาที่มีความสอดคล้องตามเกณฑ์ในด้านข้อบ่งใช้และข้อห้ามใช้ตามลำดับ เมื่อประเมินทั้งด้านข้อบ่งใช้และข้อห้ามใช้ร่วมกันพบว่ามีความสอดคล้องกับเกณฑ์เพียง 4 ใบสั่งยา(ร้อยละ1.53) สรุปผลการวิจัย: การสั่งใช้ยา Non-ED ARBs ที่สอดคล้องตามเกณฑ์ที่พัฒนาขึ้นมีไม่มากนัก นอกจากนั้นพบการสั่งใช้ยาที่ไม่สามารถประเมินได้เป็นจำนวนมาก จากผลดังกล่าวจึงควรมีการพัฒนาแนวทางของโรงพยาบาลในการสั่งใช้ยาในกลุ่ม Non-ED ARBs ให้สอดคล้องกับบริบทของโรงพยาบาล และแจ้งให้แพทย์รับทราบเพื่อประชาสัมพันธ์ให้เกิดการใช้ยาอย่างเหมาะสมในอนาคต
