การพัฒนาตัวชี้วัดคุณภาพด้านกระบวนการจ่ายยาสำหรับเภสัชกรโรงพยาบาล ในผู้ป่วยโรคข้ออักเสบรูมาตอยด์

บทคัดย่อ
บทนำ: กระบวนการจ่ายยาของเภสัชกร ซึ่งได้แก่ การทบทวนคำสั่งการใช้ยา และการให้คำแนะนำด้านยา สามารถลดและป้องกันปัญหาที่เกี่ยวกับยาได้ เพื่อให้กระบวนการดังกล่าวครอบคลุมประเด็นคุณภาพสำคัญที่ควรมีการศึกษานี้จึงมีวัตถุประสงค์เพื่อพัฒนาตัวชี้วัดคุณภาพด้านกระบวนการจ่ายยาของเภสัชกร ในผู้ป่วยโรคข้ออักเสบรูมาตอยด์ วัสดุและวิธีการวิจัย: ใช้เทคนิคเดลฟายโดยใช้แบบสอบถาม ผู้เชี่ยวชาญซึ่งประกอบด้วยอายุรแพทย์โรคข้อและรูมาติสซั่ม 4 คน อายุรแพทย์ 6 คน เภสัชกร 18 คน จากโรงพยาบาล 15 แห่ง และคณะเภสัชศาสตร์ 2 แห่ง ตอบแบบสอบถาม 4 รอบ รอบที่ 1 สร้างตัวชี้วัดจากการทบทวนวรรณกรรมและศึกษารูปแบบการใช้ยาในผู้ป่วย และมีคำถามปลายเปิดเพื่อให้ผู้เชี่ยวชาญแสดงความคิดเห็นเพิ่มเติม รอบที่ 2 ตัวชี้วัดสร้างจากคำตอบของรอบที่ 1 เป็นแบบสอบถามมาตราส่วนประเมินค่า 5 ระดับ ผู้เชี่ยวชาญจะพิจารณาว่าตัวชี้วัดมีความเหมาะสมและเป็นไปได้เพียงใด รอบที่ 3 และ 4 เป็นคำถามเดียวกับรอบที่ 2 ซึ่งแสดงค่ามัธยฐาน (Median) และพิสัยระหว่างควอไทล์ (Interquartile Range; IQR) ของคำตอบในรอบก่อนหน้า เพื่อให้ผู้เชี่ยวชาญได้ทบทวนคำตอบของตนเมื่อเทียบกับคำตอบของผู้อื่น ตัวชี้วัดจะสร้างจากฉันทามติของผู้เชี่ยวชาญในรอบที่ 4 ตามเกณฑ์มัธยฐาน > 3.5 และพิสัยระหว่างควอไทล์ < 1 ผลการศึกษา: ผู้เชี่ยวชาญตอบแบบสอบถามกลับ 24 คน (ร้อยละ 85.71) ได้ตัวชี้วัด 12 ข้อ ซึ่งครอบคลุมในกลุ่มยาต้านรูมาติสซั่มที่ปรับเปลี่ยนการดำเนินไปของโรค (DMARDs) ยาต้านอักเสบที่มิใช่ สเตียรอยด์ (NSAIDs) และยาสเตียรอยด์ (Steroids) โดยแบ่งเป็นกระบวนการให้คำแนะนำด้านยา 6 ข้อ กระบวนการทบทวนคำสั่งการใช้ยา 6 ข้อ สรุปผล: การศึกษานี้ ได้ตัวชี้วัดคุณภาพด้านกระบวนการจ่ายยาสำหรับเภสัชกรโรงพยาบาลในผู้ป่วยโรคข้ออักเสบรูมาตอยด์ ซึ่งคาดหวังว่าจะเพิ่มคุณภาพในกระบวนการจ่ายยาเพื่อป้องกันและลดปัญหาที่เกี่ยวกับยาที่อาจเกิดขึ้นทั้งนี้ควรมีการศึกษาต่อเพื่อประเมินผลของการนำตัวชี้วัดนี้ไปใช้ได้ ว่าสามารถใช้ได้จริงและมีความสัมพันธ์กับผลลัพธ์ของการใช้ยาในผู้ป่วยหรือไม่
