การประเมินการสั่งใช้ยาแวนโคมัยซินชนิดรับประทานสำหรับการรักษาการติดเชื้อคลอสตริดิออยเดส ดิฟฟิไซล์ ณ โรงพยาบาลมหาวิทยาลัย

บทคัดย่อ
การติดเชื้อคลอสตริดิออยเดส ดิฟฟิไซล์ ที่เพิ่มสูงขึ้น ส่งผลให้การใช้ยาแวนโคมัยซินชนิดรับประทานที่เป็นยาหลักในการรักษาต้องมีความเหมาะสม และให้ผลลัพธ์ทางคลินิกที่ดี เพื่อเป็นการส่งเสริมการใช้ยาอย่างสมเหตุผล การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาคุณลักษณะของผู้ป่วย แบบแผนการใช้ยาแวนโคมัยซินชนิดรับประทาน ความเหมาะสมของการสั่งใช้ยา และผลลัพธ์ทางคลินิกของการติดเชื้อชนิดนี้ วิธีดำเนินการวิจัย: การวิจัยเชิงพรรณนาแบบย้อนหลังในผู้ป่วยที่ได้รับยาแวนโคมัยซินชนิดรับประทาน ณ โรงพยาบาลศรีนครินทร์ในปีงบประมาณ 2568 เก็บข้อมูลจากฐานข้อมูลอิเล็กทรอนิกส์ จำแนกผู้ป่วยตามผลการตรวจทางห้องปฏิบัติการ ความเหมาะสมของการใช้ยา และผลลัพธ์การรักษา วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และเชิงอนุมานเพื่อเปรียบเทียบความแตกต่างระหว่างกลุ่ม ผลการวิจัย: ผู้ป่วยผ่านเกณฑ์เข้าร่วมการวิจัยรวม 145 ราย ส่วนใหญ่เป็นเพศชาย (ร้อยละ 60.7) อายุมากกว่า 65 ปี (ร้อยละ 49.0) และมีโรคประจำตัว (ร้อยละ 95.9) มีการติดเชื้อรวม 154 รอบการรักษา ส่วนใหญ่เป็นการติดเชื้อครั้งแรก (ร้อยละ 92.9) และเป็นการติดเชื้อที่ไม่รุนแรง (ร้อยละ 39.0) การใช้ยาในกลุ่มที่ควรได้รับการรักษา 99 รอบการรักษา มีความเหมาะสมทุกด้านร้อยละ 52.5 ผู้ป่วยส่วนใหญ่มีอุณหภูมิกายลดลง (ร้อยละ 90.5) และถ่ายอุจจาระลดลง (ร้อยละ 62.9) ไม่พบความแตกต่างของผลลัพธ์การรักษาระหว่างกลุ่มที่จำแนกตามผลการตรวจทางห้องปฏิบัติ หรือความเหมาะสมของการใช้ยา สรุปผลการวิจัย: การใช้ยาแวนโคมัยซินมีความเหมาะสมทุกด้านเพียงร้อยละ 52.5 แต่ผลลัพธ์การรักษาทางคลินิกอยู่ระดับที่ดี และไม่มีความแตกต่างระหว่างกลุ่มของการติดเชื้อ แต่ยังควรมีการพัฒนาแนวทางการบริหารจัดการยา เพื่อเป็นการส่งเสริมการใช้ยาอย่างสมเหตุผล
