ผลของการให้บริบาลทางเภสัชกรรมต่อคุณภาพชีวิต และความรุนแรงของการเกิดอาการไม่พึงประสงค์จากยาเคมีบำบัดในผู้ป่วยเคมีบำบัด โรงพยาบาลพระจอมเกล้าจังหวัดเพชรบุรี

บทคัดย่อ

เภสัชกรเคมีบำบัดมีบทบาทสำคัญในการให้บริบาลทางเภสัชกรรมเพื่อประเมิน ป้องกัน และบรรเทาอาการไม่พึงประสงค์จากยา (Adverse Drug Reactions: ADRs) อันจะส่งเสริมความปลอดภัยและคุณภาพชีวิต (Quality of Life: QoL) ของผู้ป่วยโรคมะเร็ง วิธีการศึกษา: การวิจัยเชิงทดลองเบื้องต้นมีวัตถุประสงค์เพื่อประเมินคุณภาพชีวิต (QoL) และความรุนแรงของอาการไม่พึงประสงค์จากเคมีบำบัดในผู้ป่วยที่ได้รับการบริบาลทางเภสัชกรรม ณ โรงพยาบาลพระจอมเกล้าจังหวัดเพชรบุรี ศึกษาในกลุ่มตัวอย่างผู้ป่วย 90 คน ที่เข้าร่วม “โปรแกรมการบริบาลทางเภสัชกรรมผู้ป่วยเคมีบำบัด” ระหว่างวันที่ 1 มกราคม ถึง 31 ตุลาคม พ.ศ. 2567 ประเมินคุณภาพชีวิตโดยใช้แบบสอบถามมาตรฐาน EQ-5D-5L เพื่อวัดค่าอรรถประโยชน์ (Utility score) และคะแนนสถานะสุขภาพโดยรวมด้วยตนเอง (Visual Analogue Scale: VAS) เปรียบเทียบผลก่อนและหลังได้รับเคมีบำบัด และใช้เกณฑ์ Common Terminology Criteria for Adverse Events (CTCAE) version 5 ในการประเมินความรุนแรงของอาการไม่พึงประสงค์ ผลการศึกษา: คุณภาพชีวิตในมิติด้านการเคลื่อนไหว การดูแลตัวเอง และการทำกิจวัตรประจำวัน ก่อนและหลังได้รับเคมีบำบัดไม่มีความแตกต่างกัน โดยหลังได้รับเคมีบำบัด ผู้ป่วยพบปัญหาด้านความเจ็บปวดหรือสบายร่างกายมากขึ้น แต่ความวิตกกังวล/ซึมเศร้ามีแนวโน้มลดลง ค่าอรรถประโยชน์ก่อนและหลังรับเคมีบำบัดแต่ละรอบไม่แตกต่างกัน ส่วนคะแนนสภาวะสุขภาพทางตรง (VAS) เพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (p=0.009) เป็น 77.01+13.11, 79.70+13.87,79.78+12.89 และ 81.04+16.41 ตามลำดับ โดยคะแนนหลังรับเคมีบำบัดรอบที่ 1-3 เพิ่มขึ้นจากก่อนรับยาเคมีบำบัด และรอบที่ 3 เพิ่มขึ้นจากรอบที่ 2 อย่างมีนัยสำคัญทางสถิติ (p<0.05) อาการไม่พึงประสงค์ระดับไม่รุนแรงที่พบบ่อย ได้แก่ ผมร่วง (ร้อยละ 74.5) อ่อนเพลีย (ร้อยละ 64.8) และชาปลายประสาท (ร้อยละ 37.8) ขณะที่พบอาการไม่พึงประสงค์รุนแรงเพียง 7 ครั้ง ได้แก่ ปวดเมื่อยกล้ามเนื้อ 5 ครั้ง และคลื่นไส้อาเจียน 2 ครั้ง สรุปผล: การบริบาลทางเภสัชกรรมช่วยเพิ่มคุณภาพชีวิตและลดความรุนแรงของอาการไม่พึงประสงค์จากเคมีบำบัด ผู้ป่วยเคมีบำบัดทุกรายจึงควรได้รับการบริบาลทางเภสัชกรรมอย่างต่อเนื่องตลอดระยะเวลาการรักษา เพื่อส่งเสริมคุณภาพชีวิต ความปลอดภัย และประสิทธิผลการรักษาในผู้ป่วยมะเร็งอย่างยั่งยืน
